运用牙周治疗仪联合中药治疗慢性牙周炎的临床研究

Clinical study of periodontal treatment apparatus combined with traditional chinese medicine in treatment of chronic periodontitis

目的探讨牙周治疗仪联合中药治疗慢性牙周炎的临床效果。方法选取本院2013年4月-2015年1月确诊的慢性牙周炎患者70例，随机分为Vector组和Gracey组，每组35例，记录治疗前各项牙周临床指标及患者的疼痛感受。Vector组在牙周基础治疗后再行中医辨证分型治疗，3个月后复查各项牙周指标。结果Vector组和Gracey组患者治疗后菌斑指数（PLI）、牙周探诊深度（PPD）、临床附着丧失（CAL）、出血指数（BI）均有明显改善，Vector牙周治疗仪和Gracey龈下刮治器在改善牙周临床指数上差异无统计学意义（P〉0．05），但Vector组患者在治疗中的疼痛程度明显小于Gracey组（P〈0．05）。Vector组施行中医辨证治疗的患者3个月后总有效率（83．3％）比常规治疗的患者（64．7％）高（P〈0．05）。结论Vector牙周治疗仪和Gracey龈下刮治器在治疗慢性牙周炎上均能达到良好的临床效果，且前者可以减少患者疼痛。同时使用中药汤剂能起到增强巩固疗效的目的，为中西医结合治疗慢性牙周炎提供了一种有效的临床治疗方案。

Objective To investigate the clinical effects of periodontal treatment apparatus combined with traditional Chinese medicine in the treatment of chronic periodontitis. Methods Seventy patients diagnosed with chronic periodon- titis in our hospital from April 2013 to January 2015 were selected and randomly divided into the Vector group and the Gracey group, with 35 patients in each group. The periodontal clinical indicators and the patients＂ feeling of pain before treatment were recorded. The Vector group was given Chinese medicine syndrome differentiation treatment after peri- odontal non-surgical treatment and the periodontal indicators were rechecked 3 months later. Results After treatment, plaque indices （PLI）, periodontal probing depth （PPD）, clinical attachment loss （CAL） and bleeding indices （BI） of both the Vector group and the Gracey group improved significantly. The Vector periodontal treatment apparatus and the Gracey subgingival scaling apparatus were not statistically different in the improvement of periodontal clinical indices （P〉0.05）; The Vector group was significantly lower than the Gracey group in the degree of pain during treatment （P〈 0.05）. The total effective rate （83.3%） of the patients of the Vector group receiving Chinese medicine differentiation treatment was higher than that（64.7%） of those receiving conventional treatment （P〈0.05）. Conclusion Both the Vector periodontal treatment apparatus and the Gracey subgingival scaling apparatus show favorable clinical effects in the treatment of chronic periodontitis and the former one can reduce the patients＂ pain. The simultaneous application of Chinese medicine decoction can enhance and solidify the efficacy and provide an effective clinical treatment plan for the integrated Chinese and Western medicine treatment of chronic periodontitis.
