家庭健康护理计划对产妇角色适应能力和自理能力的影响

目的探讨家庭健康护理计划对产妇角色适应能力和自理能力的影响。方法采用随机数字表法将我院2014年10月～2015年2月收治的120例初产妇分为对照组和观察组，观察组实施家庭健康护理计划；对照组实施常规护理，并观察两组初产妇角色适应能力、自理能力及母乳喂养自我效能等。结果观察组初产妇自理能力优秀、良好率分别为70．00％、26．67％，角色适应能力良好、中等分别为38．33％、50．00％，对照组初产妇自理能力优秀、良好率分别为28．33％、30．00％；角色适应能力良好、中等分别为28．33％、41．67％，观察组自理能力及角色适应能力均明显高于对照组（P〈0．05），产后1周和产后4个月观察组初产妇母乳喂养自我效能及知识得分明显高于对照组（P〈0．05）；观察组产妇剖宫产及抑郁症发生率（5．00％，0）明显低于对照组（16．67％，6．67％）（P〈0．05）。结论家庭健康护理计划在初产妇护理中应用，不仅能够有效提高产妇角色适应能力和自理能力．同时对提高初产妇母乳喂养自我效果，降低抑郁及剖宫产等情况也具有重要的作用，值得临床推广使用。

Objective To investigate the effect of adaptability and self-care ability of home health care program treat- ment of puerpera. Methods 120 cases of primipara treated in our hospital from October 2014 to February 2015 were randomly divided into observation group and control group with random number table, and the observation group was given home health care program,while the control group was given routine care. Role adaptability, self-care ability and breastfeeding self-efficacy of the two groups were observed. Results Excellent, good rate of self-care ability of observa- tion group were respectively 70.00%, 26.67%, Good, medium rate of role adaptability were respectively 38.33%, 50.00%, Excellent, good rate of self-care ability of control group were respectively 28.33% ,41.67%,Good ,medium rate of role adaptability were respectively 28.33%,30.00%, self-care ability and role adaptability of observation group were signifi- cantly higher than control group（P〈0.05）. Breastfeeding self-efficacy and knowledge score of observation group in post- partum 1 week and 4 months were significantly higher than control group （P〈0.05）; The incidence of cesareansection and depression of observation group were 5.00% and 0 ,which were significantly lower than 16.67% and 6.67% of the control group （P〈0.05）. Conclusion Application of home health care program in primipara ,not only can effectively im- prove role adaptation and self-care ability,but also play an important role in improving Breast self-effects, reducing de- pression and cesarean section and is worthy of popularization.
