胫骨髓内钉入口环钻取骨植骨结合微创钢丝复位固定治疗胫骨骨折的临床效果

Clinical effects of tibial fracture by tibia intramedullary nail entry trephine took bone graft combine with minimally invasive wire reduction and fixation

目的探讨胫骨髓内钉人口环钻取骨植骨结合微创钢丝复位固定治疗胫骨骨折的临床效果。方法选取2012年6月～2014年12月我院收治的78例胫骨骨折患者作为研究对象。按照治疗方法不同将78例患者分为实验组46例和对照组32例。实验组患者采取胫骨髓内钉入口环钻取骨植骨结合微创钢丝复位固定治疗，对照组患者采取LCP钢板治疗。比较两组患者住院时间、住院费用、手术时间、关节屈曲度、伸直度、踝关节背曲度、跖屈度、骨折愈合时间及并发症发生情况。结果实验组患者住院时间、住院费用均明显低于对照组,差异有统计学意义（P〈0．05）；实验组患者手术时间明显高于对照组，差异有统计学意义（P〈0．05）。实验组患者关节屈曲度、伸直度、踝关节背曲度、跖屈度、骨折愈合时间均优于对照组，差异有统计学意义（P〈0．05）。实验组患者并发症总发生率明显低于对照组，差异有统计学意义（P〈0．05）。结论胫骨髓内钉入口环钻取骨植骨结合微创钢丝复位固定治疗胫骨骨折临床效果较好，可有效缩短患者住院时间，减少患者住院费用，对关节屈曲度、伸直度、踝关节背曲度、跖屈度、骨折愈合时间效果较好，而且术后并发症发生率较低，值得临床推广。

Results The hospital stays, hospitalization expenses for the experiment group were lower than the control group（P〈0.05）; The time of operation for the experiment group were higher than the control group （P〈0.05）; There were significant differences on the index of joint flexion degree, straight Mess, curvature of the ankle, plantar flexion degree, fracture healing time（P〈0.05）; The rate of complication for the experiment group were lower than the control group（P〈0.05）. Conclusion It has good clinical effects for the patients with tibial fracture by tibia intramedullary nail entry trephine took bone graft combine with minimally invasive wire reduction and fixation, it can reduce the time of hospital stays, save hospitalization expenses, with no influence on the index of joint flexion degree, straight Mess, curvature of the ankle, plantar flexion degree, fracture healing time, with lower complication. It is worthy of clinical promotion.
