肱骨近端锁定钢板治疗老年骨质疏松性肱骨外科颈骨折

A locking proximal humeral plate in treatment of senile osteoporosis humerus surgical neck fracture

目的探讨经肌间隙入路应用肱骨近端锁定钢板（locking proximal humerus plate,LPHP）治疗老年骨质疏松性肱骨外科颈骨折的临床疗效。方法选取2011年8月-2014年10月62例患有肱骨外科颈骨折的老年患者,随机分为对照组和治疗组,对照组26例、治疗组36例。对照组采用普通解剖钢板内固定技术;治疗组采用肱骨近端锁定加压钢板经三角肌胸大肌间隙入路实施治疗。记录两组患者的手术效果、手术时间、术中出血、住院时间、骨折愈合时间等情况,对相关数据进行统计学分析。结果治疗组治疗效果明显优于对照组（P〈0.05）;治疗组手术时间、术中出血、住院时间、骨折愈合时间均优于对照组（P〈0.05）。结论对于老年骨质疏松性肱骨外科颈骨折采用肱骨近端锁定钢板经三角肌胸大肌间隙入路治疗可取得良好疗效。

Objective To observe the clinical curative effect of application of locking proximal humeral plate（LPHP）from the muscular clearance in treatment the senile osteoporotic humerus surgical neck fracture.Methods From August2011 to October 2014,62 cases of elderly patients with humerus surgical neck fracture were selected randomly divided into control group and treatment group,control group with 26 cases,treatment group of 36 cases.Adopt common anatomic plate internal fixation treatment technology was given to the control group patients;Adopt proximal humerus locking compression plate by the deltoid pectoralis clearance in the treatment for the treatment group.Two groups of patients with surgical effect,operation lime,intraoperative bleeding,hospitalization time,fracture healing lime,etc.were recorded and the related data were analyzed.Results The treatment group therapy effect was better than the control group（P〈0.05）;Operation time,intraoperative bleeding,hospitalization time,fracture healing time were better than the control group（P〈0.05）.Conclusion Adopt proximal humerus locking compression plate by the deltoid pectoralis clearance in treating the senile osteoporotic humerus surgical neck fracture can achieve a good clinical outcome.
