摘要 目的讨论布氏杆菌性脊柱炎的影像学特征,从而提高对该病的影像认知。方法搜集2012年3月-2014年9月12例布氏杆菌性脊柱炎患者的CT、MRI资料分析并回顾文献。结果 10例病变位于腰椎,2例位于胸椎。12例中病变椎体均有不同程度骨质增生。病灶周围骨质增生硬化,病变均为多个椎体受累,受累椎体骨质密度不均匀增高。其中3处相邻椎间隙变窄。1例病变椎体周围可见软组织密度,推挤相邻腰大肌。MR上,病灶呈T1低信号,T2高信号、低信号或混杂信号,受累椎间盘呈低信号改变,骨质硬化处信号减低。布氏杆菌性脊柱炎以下腰椎感染常见,受累椎体多伴有明显骨质增生。结论在螺旋CT及MR图像上,布氏杆菌性脊柱炎具有特异的影像表现。 Objective To discuss the imaging characteristics of burcellar spondylitis,so as to improve the understanding of the imaging of this disease.Methods CT and MRl data analysis of 12 patients with burcellar spondylitis from March2012 to September 2014 were collected,and the literature was reviewed.Results 10 cases of lesions were located at lumbar vertebra,and 2 cases were located at thoracic vertebra.Among the 12 cases,there were different degrees of hyperostosis in the vertebrae with lesions.Hyperostosis and osteosclerosis was observed around the lesions.Multiple vertebrae were affected due to the lesions,and the bone density of the affected vertebrae was unevenly increased.The intervertebral spaces between 3 adjacent vertebrae were narrowed.Soft tissue density could be observed in one vertebrae with lesions,and adjacent psoas major was squeezed.In the MR,T1 low signal,T2 high signal and low signal or mixed signal were shown in the lesions.Low signal change was shown in the affected intervertebral disc,and the signal was decreased at the location of osteosclerosis.Infection of lumbar vertebrae was common in burcellar spondylitis,and the affected vertebrae was accompanied with obvious hyperostosis.Conclusion In the images of spiral CT and MR,the burcellar spondylitis has specific imaging characteristics.
