住院危重新生儿肺炎克雷伯菌定植的影响因素研究

Influence factors of critically ill hospital newborn with klebsiella pneumoniae colonize

目的研究住院危重新生儿肺炎克雷伯菌定植的影响因素。方法选择在我院治疗的肺炎克雷伯菌定植的新生危重患儿80例为定植组，同期无肺炎克雷伯菌定植的新生危重患儿80例为对照组。比较两组患儿的临床资料。结果发现住院危重新生儿肺炎克雷伯菌定植与出生胎龄、出生体重、住院天数、呼吸机使用时间、合并先天性心脏病、静脉营养时间、经外周静脉穿刺中心静脉置管、外科手术、使用碳青霉烯类抗生素、使用糖肽类抗生素、碳青霉烯类抗生素使用时间有关，差异具有统计学意义（P〈0．05）。显示碳青霉烯类抗生素使用时间是住院危重新生儿肺炎克雷伯菌定植的独立危险因素（OR=2．373，95％CI=1．276—4．413，P=0．006）。结论较长时间使用碳青霉烯类抗生素是影响住院危重新生儿肺炎克雷伯菌定植的独立危险因素。

Objective To study the influence factors of critically ill hospital new-born klebsiella bacteria colonize. Methods 80 cases of critically ill newborn with klebsiella pneumoniae colonize who stayed in our hospital for treatment from January 2012 and January 2015 were selected as the engraftment group, and 80 cases of new critically ill children without pneumonia klebsiella bacteria colonize at the same time were selected as the control group. Their clinical data were compared between the two groups to analyze the influencing factors by single factor analysis and Logistic regression. Results It was found that the hospital critically ill newborn with pneumonia klebsiella bac- teria colonize was related with gestational age, birth weight, hospital stays, breathing machine use time, merging, congenital heart disease, intravenous nutrition, via peripheral venous puncture center venipuncture, surgery, carbon penicillium use, sugar peptide antibiotics use, and carbon alkene antibiotics penicillium alkene on antibiotic use time （ P 〈 0.05 ）. Taking the above factors into Logistic muhivariate regression analysis, it showed that the carbon penicillium alkene antibiotic use time was the independent risk factor of hospital critically ill newborn with klebsiella pneumonia bacteria colonize （ OR = 2. 373, 95% CI = 1. 276 - 4. 413, P = 0. 006）. Conclusion The long time use of carbon penicillium alkene antibiotics is the independent risk factor of critically ill newborn with pneumonia klebsiella bacteria colonize.
