中药联合三联疗法对幽门螺旋杆菌的根除率以及再感染的影响

Effect of Applying Traditional Chinese Medicine Combined with Triple Therapy in the Eradication Rate of Helicobacter Pylori and its Re-infection

目的：观察中药汤剂黄连解毒汤联合三联疗法对幽门螺旋杆菌的根除率以及再感染的影响。方法：将300例幽门螺旋杆菌感染患者按照1：1随机分为对照组和观察组,对照组采用奥美拉唑20mg、阿莫西林100mg、克林霉素500mg进行治疗,每天2次;观察组在对照组治疗的基础上加用黄连解毒汤。两组均治疗7天,4周后进行13CBUT试验检测。行3年随访,观察复发情况。结果：观察组和对照组幽门螺旋杆菌根除率分别为86%和70%,统计分析有显著性差异（P〉0.05）;两组总有效率分别为96%、85.3%。对照组第1~3年幽门螺旋杆菌累积再感染率分别为4.76%、5.93%、8.53%,观察组幽门螺旋杆菌累积再感染率分别为1.36%、2.21%、8.46%,两组比较,在随访第1年、第2年两组的复发情况有显著差异（P〈0.05）。结论：黄连解毒汤联合三联疗法用于幽门螺旋杆菌根除安全而有效。提高幽门螺旋杆菌的根除率可有助于减少幽门螺旋杆菌的复发感染。

Objective： To observe the eradication rate of traditional Chinese medicine decoction of Huanglian Jiedu Decoction combined with triple therapy for helicobacter pylori and the effect of re-infection. Methods： 300 cases of helicobacter pylori infection in patients in 1： 1were randomly divided into controlled group and observation group,the controlled group used omeprazole 20 mg,amoxicillin 100 mg,clindamycin 500 mg treatment,2times a day; the observation group treated with Huanglian Jiedu Decoction on the basis of the controlled group. The two groups were treated for 7days,13C-BUT test was done after4 weeks of treatment. 3years follow-up observation,relapse situation was noticed. Results： The eradication rates of the observation group and the controlled group respectively were 86% and 70%,statistical analysis had significant difference（ P〉0. 05）;the total effectiveness of the two groups respectively were 96% and 85. 3%. The 1 ~ 3year cumulative and helicobacter pylori infection rates of the controlled group respectively were 4. 76%,5. 93% and 8. 53%,helicobacter pylori cumulative re-infection rates of the observation group were 1. 36%,2. 21%,8. 46%,two group of comparison,during first years of follow-up,recurrence second years of the two groups had significant difference（ P〈0. 05）. Conclusion： Huanglian Jiedu Decoction combined with triple therapy for helicobacter pylori eradication is safe and effective. Increasing eradication rate of helicobacter pylori can help reduce the recrudescence of helicobacter pylori infection.
