冠心病患者的炎性指标检测与红细胞分布宽度、B型钠尿肽的相关研究 被引量:6

摘要 目的探讨白细胞(WBC)、超敏C反应蛋白(hs-CRP)等炎性指标及红细胞分布宽度(RDW)、B型脑钠肽(BNP)与冠状动脉粥样硬化性心脏病的关系,观察上述指标之间有无依从性,以明确其作为冠心病危险因素预测指标的应用价值及临床意义。方法筛选住院冠心病患者90例,分为急性心肌梗死(AMI)组和心绞痛组,选同期健康体检者45例为对照组,均采集空腹静脉血,检测并记录RDW、BNP、hs-CRP、WBC及中性粒细胞(NEU)的数值,进行统计学分析。结果 AMI组上述各指标的均值均高于对照组,差异有统计学意义(P<0.05);心绞痛组的WBC、NEU均值与对照组比较差异无统计学意义(P>0.05);冠心病患者RDW与BNP的变化呈正相关(r=0.947),与hs-CRP的变化也呈正相关(r=0.926);3个年龄段的冠心病患者BNP均值间比较差异有统计学意义(P<0.05)。结论炎性指标WBC尤其是NEU,只有在AMI时才明显升高,而在组织损伤较轻的心绞痛患者变化不明显;冠心病患者RDW水平升高与体内BNP、hs-CRP浓度升高呈同步变化,RDW可作为诊断冠心病及分型的良好指标;RDW、WBC的检测便捷、廉价,临床应用前景广泛。
