ALDH1作为肺癌干细胞功能标志物的研究

Study on ALDH1 as the functional marker of lung cancer stem cells

目的探讨乙醛脱氢酶-1(ALDH1)作为肺癌干细胞标志物在肺癌中的表达情况及临床意义。方法采用免疫组化法检测44例肺癌组织及癌旁组织中ALDH1的表达情况,分析ALDH1表达情况与临床病理特征、无病生存率的相关性,应用COX比例风险模型分析预后影响因素。结果肺癌组织中ALDH1阳性表达率明显高于癌旁组织(P<0.05)。ALDH1表达情况与肺癌患者的年龄、性别、肿块大小、吸烟指数、组织学类型和淋巴结转移无明显相关性(P均>0.05),与TNM分期与分化程度明显相关(P均<0.05)。ALDH1阳性表达患者无病生存率明显低于阴性表达者(P<0.05)。多因素COX比例风险模型分析显示ALDH1表达与TNM分期是影响患者预后的独立危险因素(P均<0.05)。结论 ALDH1在肺癌组织中呈现高表达,表达差异与TNM分期及分化程度相关;ALDH1阳性表达患者预后较差,与TNM分期同为患者预后的独立危险因素。检测ALDH1的表达情况对判断患者的预后有重要意义。

Objective It is to investigate the expression and clinical significance of ALDH1 as a marker of lung cancer stem cells in lung cancer. Methods The expressions of ALDH1 in tissue adjacent to carcinoma, and lymph node metastasis in 44 cases of patients with lung cancer were detected by immunohistochemical method. The relevance of ALDH1 expressions of patients with lung cancer with clinicopathologic features and Disease-free survivals （DFS） were analyzed, and the prognostic influence factors were analyzed by COX proportional hazard model. Results The positive expression rate of ALDH1 in cancer tissues was markedly higher than that of tissue adjacent to carcinoma （ P 〈 0.05 ）. There was no significant correlation between the ALDH1 expression with age, gender, tumor size, smoking index, histological type and lymph node metastasis （all P 〉 O. 05 ）, but was significant correlation between the ADHLI expression with TNM staging and its differentiation （ all P 〈 0.05 ）. The DFS of patients with positive expression of ALDH1 were markedly shorter than that of ones with negative expression （P 〈 0.05 ）. The results of analysis of muhivariable COX model showed that the ALDH1 expression and TNM staging were the inde- pendent factors influencing the prognosis of patients （ all P 〈 0.05 ）. Conclusion ALDH1 shows a high expression in lung cancer tissues, and expression differences are associated with TNM stage and differentiation. ALDH1 patients with positive ex- pression have poorer prognosis, and ALDH1 expression and TNM staging are independent factors affecting prognosis of pa- tients. So it is important and significant to observe ALDH1 expression in clinic.
