胃镜下注射联合钛夹治疗急性上消化道出血的临床疗效

Effect of combining epinephrine injection under gastroscope with titanium clip in the treamtent of acute upper gastrointestinal hemorrhage

目的探讨胃镜下注射联合钛夹治疗急性上消化道出血的临床疗效。方法将我院收治的64例急性上消化道出血患者按照随机分层分组法分为观察组和对照组,对照组胃镜下注射肾上腺素治疗,观察组在此基础上联合钛夹治疗,观察两组止血情况、输血量、再出血率、住院时间、血清C反应蛋白(CRP)及皮质醇水平。结果:观察组止血时间、即时止血率及再出血率均优于对照组,两组差异有统计学意义(P<0.05)。观察组输血量、住院时间、血清CRP及皮质醇水平均优于对照组,两组差异有统计学意义(P<0.05)。结论胃镜下注射联合钛夹治疗急性上消化道出血疗效显著,可显著缩短出血时间,预防再次出血,在急性上消化道出血的治疗中具有重要应用价值。

Objective ：To explore the effect of combining epinephrine injection under gastroscope with titanium clip inthe treatment of acute upper gastrointestinal hemorrhage. Methods ： According to a stratified randomization method, 64 ca-ses of patients with acute upper gastrointestinal hemorrhage were divided into the observation group and the control group,the control group was given epinephrine injection, the observation group was given epinephrine injection combined with tita-nium clips, two groups of bleeding time, the volume of blood transfusion, re - bleeding rate, hospitalization time, the ser-um levels of CRP and cortisol. Results ： The observation group was significantly better than the control group in the bleedingtime, the instant hemostasis rate and the rehaemorrhagia rate, there are significant differences between the two groups （P 〈0.05 ）. The observation group was significantly better than the control group in the volume of blood transfusion, hospitaliza-tion time, the serum levels of CRP and cortisol, there are significant differences between the two groups（P 〈0.05 ）. Con-clusion： Combined epinephrine injection under / astroscope with titanium clip can significantly shorten the bleeding timeand prevent re - bleeding, it has worthy of clinical application and promotion.
