贝伐单抗联合顺铂腔内灌注治疗恶性胸腹水的临床研究

Clinical study of bevacizumab combined with cisplatin intracavitary perfusion in the treatment of malignant hydrothorax and ascites

目的观察贝伐单抗联合顺铂腔内灌注治疗恶性胸腹水的临床效果。方法选择2013年3月—2014年9月收治的60例恶性胸腹水患者作为研究对象,随机分为对照组和观察组各30例。对照组给予顺铂腔内灌注方案,观察组接受贝伐单抗联合顺铂腔内灌注方案。比较两组患者的治疗效果、不良反应发生情况及生活质量评分。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果观察组整体缓解率为80.0%,对照组为40.0%,两组比较差异有统计学意义(P<0.05)。观察组治疗后生活质量评分为(46.8±2.6)分,明显优于对照组的(35.9±1.2)分,差异有统计学意义(P<0.05)。结论在恶性胸腹水患者的临床治疗中,采用贝伐单抗联合顺铂腔内灌注方案,可提升临床治疗效果,优化患者的生活质量,提高治疗的安全性,值得临床推广应用。

Objective To observe the clinical effect of bevacizumab combined with cisplatin intracavitary perfusion in the treatment of malignant hydrothorax and ascites. Methods 60 patients with malignant hydrothorax and ascites from March 2013 to September 2014 were selected and randomly divided into control group and observation group（n=30）. The patients in control group were given cisplatin ntracavitary perfusion scheme and the patients in observation group received bevacizumab combined with cisplatin ntracavitary perfusion scheme. The therapeutic effects, occurrence of adverse reactions and quality of life score were compared between the two groups. The measurement data was processed by t test, count data was processed by chi square test,P〈0.05 was considered statistically significant. Results The overall remission rate of observation group was 80.0%, in the control group was 40.0%, the difference was statistically significant（P〈0.05）. The quality of life scores in the observation group after treatment was（ 46.8±2.6） points, which was significantly better than the control group[（ 35.9±1.2） points],the different was statically significant（P〈0.05）. Conclusion In the clinical treatment of patients with malignant hydrothorax and ascites, to adopt bevacizumab combined with cisplatin intracavitary perfusion scheme can improve the clinical treatment effect, optimize the quality of life of patients and improve the safety of the treatment. It is worthy of promotion.
