摘要
目的:探究血清红细胞生成素(EPO)水平在深Ⅱ度烧伤创面的临床作用。方法:在我院中度烧伤治疗61例患者中随机选择深Ⅱ度烧伤创面120个,根据患者EPO水平的中位数倍数(MOM)分为三组,另选择40例健康体检者作为对照组;评价并比较各组血清EPO水平及三组患者创面的愈合时间、6个月后创面面积的愈合率以及瘢痕评分。结果:烧伤12d后患者血清EPO水平为(19.55±8.37)U/L,显著高于对照组的(7.78±3.29)U/L(t=8.658,P<0.001);三组患者间的创伤面愈合时间、6个月愈合率以及患者6个月瘢痕VSS评分的色泽、厚度、柔软度及血管分布均存在显著差异(P<0.05);且治疗6个月后患者深Ⅱ度烧伤创面愈合率与血清EPO水平呈现正相关关系(r=0.35,P<0.05);血清EPO水平与患者创伤愈合时间、患者6个月瘢痕VSS评分的色泽、厚度、柔软度及血管分布评分均呈现负相关关系(r=-0.29,r=-0.38,r=-0.47,r=-0.45,r=-0.49;P<0.05)。结论:血清EPO水平可以促进中度烧伤患者深Ⅱ度烧伤创面的愈合,缩短创伤面恢复的时间,提高愈合及愈合质量,在临床实践中值的推广应用。
Objective: To explore the clinical effect of the serum erythropoietin(EPO) level in deep Ⅱ degree burn wounds.Methods: Selected 120 wounds of deep Ⅱ degree burns randomly from 61 cases of patients with moderate burns who were treated in our hospital, then divided into three groups according to the median multiples of serum EPO level(MOM), and another 40 cases of healthy physical examination were chose as control group; Evaluated and compared the serum EPO level of all groups, and the wound healing time, wound healing rate 6 months after treatment and wound scar VSS score 6 months after treatment among three groups of patients.Results: Serum EPO level in patients 12 d after treatment were(19.55+8.37) U/L, which was significant higher than(7.78±3.29)U/L in control group(t=8.658, P〈0.001); The wound healing time,wound healing rate 6 months after treatment and the color, thickness, softness and vascular distribution VSS score of wound scar 6 months after treatment were significant different among the three groups(P〈0.05);Healing rate 6 months after treatment was positive correlated with the serum EPO level(r=0.35, P〈0.05); Wound healing time and the color, thickness, softness and vascular distribution VSS score of wound scar 6 months after treatment were negative correlated with the serum EPO level(r=-0.29, r=-0.29, r=-0.47, r=-0.45, r=-0.49, P〈0.05). Conclusion: High serum EPO level can promote the wound healing in deep Ⅱ degree burn wounds of moderately burn patients, it is able to shorten the healing time, improve the wound healing rate and quality, which should be applied widely in clinical practice.
