开腹与腹腔镜子宫次全切除术治疗子宫肌瘤效果比较

Effective comparison of subtotal hysterectomy between laparotomy mode and laparoscopic mode in treating uterine leiomyoma

目的探讨开腹与腹腔镜子宫次全切除术治疗子宫肌瘤临床疗效差异。方法选取该院2012年1月至2015年6月收治子宫肌瘤患者130例,分为A组和B组,各65例。A、B组分别采用开腹术式和腹腔子宫次全切除术式治疗。比较两组患者围术期临床指标、伤口甲级愈合率及术后并发症发生率等。结果两组患者手术时间比较,差异无统计学意义(P>0.05);B组患者术中出血量、尿管留置时间、肛门排气时间、下床活动时间及镇痛药使用率均显著优于A组,差异有统计学意义(P<0.05);B组患者切口甲级愈合率显著高于A组,术后并发症发生率显著低于A组,差异均有统计学意义(P<0.05)。结论相较于开腹术式,腹腔镜子宫次全切除术式治疗子宫肌瘤可以有效地减轻术中创伤,缓解术后疼痛,加快术后康复进程,且有助于降低术后并发症发生率。

Objective To investigate the clinical effects differences of subtotal hysterectomy between laparotomy opera-tion and laparoscopic operation in the treatment of uterine leiomyoma. Methods 130 patients with uterine leiomyoma in our hospital from January 2012 to June 2015 were selected and randomly divided into the group A and B,65 cases in each group. The group A and B respectively adopted the laparoscopic mode and laparotomy mode for conducting subtotal hysterectomy. The peri-operative clinical indicators,wound healing rate with class A and occurrence rate of postoperative complication were compared be-tween the two groups. Results There was no statistically significant difference in the operation time between 2 groups（P〈 0.05）;the group B was significantly superior to the group A in the aspects of the intraoperative bleeding amount,indwelling time of urinary catheter,anus exhaust time,time of activity for leaving bed, and use rate of analgesic drugs,the differences were statistically significant（P〈 0.05）;the wound healing rate with class A in the group B was significantly higher than that in the A group,while the occurrence rate of postoperative complications in the group B was significantly lower than that in the group A,the difference was statistically significant（P〈 0.05）. Conclusion Compared with the laparotomy mode,laparoscopic subtotal hysterectomy in the treatment of uterine leiomyoma can efficiently decrease the intraoperative trauma,relieve the postoperative pain and conduces to reduce the probability of the postoperative complications occurrence.
