社区健康管理双向互动模式的研究与实践

The Research and Practice of Interactive Service Model in Community Health Management

社区卫生服务中心开展健康管理是卫生事业重心下移、关口前移,做好健康促进工作的重要举措。社区卫生服务中心和居民作为供需两方,健康管理工作的开展落实困难,存在系统性和综合性的难题。本文通过对已初步建立双向互动模式的社区卫生服务中心进行阶段性总结,探讨分析开展社区健康管理服务的可行模式,为我国社区健康管理的发展提供有益的借鉴。

Implementing health management in community health service center was a crucial way to move the center of health business,move the critical pass forward and work well in health improvement.Community health service centers and residents,as the supply and demand sides,had systematic and comprehensive problem in health management implementation.In order to provide a useful reference for the development of community health management in China,it summed up the experience of the community health service center which established two-way interactive service mode and made discussion on this feasible mode of community health management services.
