超声测量联合刻度环钻辅助深板层角膜移植术

Deep lamellar keratoplasty assisted with A-ultrasonic pachymetry and scaled trephine

目的探讨A超测量联合刻度环钻辅助深板层角膜移植术治疗角膜白斑的方法及临床效果。方法对内皮层正常的角膜白斑进行深板层角膜移植术8例（8眼），术前A超测量病变角膜的厚度及术中使用刻度环钻辅助完成手术，观察术中并发症、术后角膜植片情况及视力。结果8例手术均顺利完成，1眼术中发生小穿孔，未发生植床穿透需要改行穿透性角膜移植术的情况。角膜植片透明，1例术后出现双前房，进行前房注气后好转。术后末次随访视力为0．2～0．8，较术前的手动～0．04有显著提高。结论A超测量联合刻度环钻辅助深板层角膜移植术治疗角膜白斑是安全有效的，把握好适应证，减少手术并发症。

Objective To investigate the methods and clinical effects of deep lamellar keratoplasty assisted with A-ultrasonic paehymetry and scaled trephine for corneal leucoma. Methods Deep lamellar keratoplasty assisted with A-ultrasonic pachymetry and scaled trephine was performed on 8 eyes of 8 patients with normal function of cornea endothelium. The complications during the operation, visual acuity and grafted cornea were observed, and the surgical techniques were analyzed at last. Results The operation of deep lamellar keratoplasty was performed successfully on eight eyes. In one case small rupture of decement＇ s membrane was observed during the operation and it didn＇ t affect the operation. Double anterior chamber was observed in one case and disappeared after air injection into anterior chamber. The visual acuity after operation was better than that before the operation. The visual acuity at the latest visit ranged from 0.2 to 0.8, and was significantly better than that before the operation which ranged from hand movement to 0. 04. Conclusion Deep lamellar keratoplasty assisted with A-ultrasonic pachymetry and scaled trephine is safe and effective for corneal leucoma.
