部分剪除法取代切除手术治疗结膜淋巴管扩张

Partial cut with scissors replacing the excision surgery for the treatment of conjunctival lymphangiectasis

目的探讨一种取代传统切除手术治疗结膜淋巴管扩张的新方法。方法我院2009年11月至2014年6月结膜淋巴管扩张症70例（70眼），均为初发病例，随机分为两组：试验组35例（35眼），表麻下用角膜剪行部分剪除法；对照组35例（35眼），采用局麻下行切除手术。两组术后均随访3个月，观察术后复发及术眼异物感情况。结果试验组术后3个月内有5例复发，复发率14．29％，术后3～5d异物感消失；对照组3个月内有2例复发，复发率5．71％，术后7～10d异物感消失，两组相比，复发率差异无统计学意义，异物感消失时间差异有统计学意义。结论部分剪除法治疗结膜淋巴管扩张症与手术切除法疗效相当，但异物感消除更快，手术操作更简便。

Objective To investigate a new method for conjunctival lymphangiectasis to replace the traditional surgical excision. Methods Seventy patients were diagnosed as conjunctival lymphangiectasis from Nov 2009 to Jun 2014. They were all primary cases and were randomly divided into two groups. Thirty-five cases in clinical trial group received partial resection by corneal scissors under surface anesthesia. The other 35 patients in control group received the surgical excision to remove conjunctival lymphangiectasia under local infiltration anesthesia . Followed-up of 3months, the postoperative recurrence and ocular foreign body sensation were observed. Results Five cases of the clinical trial group recurred and the recurrence rate was 14.29%. The foreign body sensation disappeared in 3 to 5 days after the surgery. Two cases of the control group recurred and the recurrence rate was 5.71%. The foreign body sensation disappeared in 7 to 10 days after the surgery. There was no statistically significant difference in recurrence rate between two groups（ P 〉 0.05 ）, but the difference in the time of foreign body sensation dissapering was statistically significant （ P 〈 0.05）. Conclusion Patial resection has the same effect compared with surgical excision for conjunctival lymphangiectasia , with shorter foreign body sensation disappearing time and more convenient surgical procedures.
