综合干预对高龄股骨近端防旋髓内钉内固定患者围手术期深静脉血栓形成的预防作用

The Effect of Comprehensive Intervention on Prevention of Deep Venous Thrombosis in the Perioperative Period of Elderly Patients with Proximal Femoral Nail Antirotation Internal Fixation

目的 探讨预防高龄股骨粗隆间骨折患者股骨近端防旋髓内钉（ PFNA ）内固定围手术期发生深静脉血栓形成（ DVT）的综合干预对策. 方法 对2013年3月~2015 年2月收治的高龄股骨粗隆间骨折行PF-NA内固定的22例患者,根据患者全身情况进行评估,对DVT发生采取了综合干预措施. 对照分析2011年3月~2012年12月同类住院手术病人15例,单纯使用基本预防措施进行处理. 结果 观察组1例病人发生胫后静脉血栓形成并不完全阻塞,无1 例患者出现脑、肺栓塞或者下肢坏死的情况. 对照组确诊DVT 3例. 结论 通过对高龄PFNA内固定患者,采取综合措施进行干预,可以有效降低高龄PFNA内固定患者围手术期DVT的发生.

Objective To explore comprehensive intervention measures which could be used to prevent Deep Venous Thrombosis （ DVT） in the Perioperative Period of Elderly Patients with intertrochanteric fracture and Proximal femoral Nail Antirotation （ PFNA） Internal Fixation.Methods Twenty-two elderly patients admitted to hospital from March 2013 to February 2015,with intertrochanteric fracture and PFNA internal fixation were enrolled in the experiment ,and according to the patient＆#39;s general condition ,the comprehensive intervention meas-ures were taken to prevent DVT .The control group was made up of fifty similar operation patients who were admitted to hospital from Decem-ber 2012 to March 2011,using of the basic intervention measures .Results In the observation group ,only one patient was with posterior tibial vein thrombosis and not completely blocked ,no one of patients was with cerebral ,pulmonary embolism or lower limb necrosis .But in the con-trol group,three cases were diagnosed as DVT .Conclusion Comprehensive Intervention on preventing deep venous thrombosis in the periop-erative period of elderly patients with PFNA Internal Fixation can effectively reduce the incidence of DVT in elderly patients with PFNA .
