经皮椎体成形术在无神经损伤及明显后凸畸形骨质疏松性胸腰椎骨折不愈合中的应用 被引量:3

摘要 目的 探讨经皮椎体成形术治疗无神经损伤及明显后凸畸形骨质疏松性胸腰椎骨折不愈合的临床效果. 方法回顾性分析2012年1月~2014年3月收治的15例骨质疏松性胸腰椎骨折不愈合但不合并神经损伤且后凸畸形〈30°患者的临床资料,所有患者均采用经皮椎体成形术治疗,采用痛觉视觉模拟评分( VAS) 及脊柱ODI评分评估手术治疗效果. 结果 15例患者VAS术前评分为(7.4±1.5)分,术后2d 所有患者评分为(1.5±0.4)分,两者比较差异有统计学意义(P〈0.05),术后3个月患者VAS评分为(1.9±0.6)分,与术后2d比较,差异无统计学意义(P〈0.05). 15例患者术前脊柱ODI评分平均64.5%,术后3个月为30.8%,两者比较差异有统计学意义(P〈0.05). 结论 经皮椎体成形术是一种治疗无神经损伤及明显后凸畸形骨质疏松性胸腰椎骨折不愈合的较好的方法. Objective To investigate the clinical efficacy of percutaneous vertebroplasty in the treatment of osteoporotic thoraco -lumbar vertebral fracture nonunion without neurological deficit and obvious kyphotic deformity .Methods From January 2012 to March 2014, the clinical data of 15 patients with nonunion of osteoporotic thoracolumbar vertebral fractures nonunion without neurological deficit and obvi -ous kyphotic deformity wereretrospective analyzed .All patients were treated by percutaneous vertebroplasty .The curative effect was evaluated by visual analogue scale ( VAS) and oswestry disability index ( ODI) .Results The VAS scores of 15 patients was 7.4±1.5 before operation (1.5±0.4) at the two days postoperatively ,there were significant difference between the two phase (P〈0.05),and(1.9±0.6) after three months,the difference was no statistically significant (P〈0.05).The average ODI score of 15 patients was 64.5%before operation,30.8%after three months,there were significant difference between the two phase (P〈0.05).Conclusion Percutaneous vertebroplasty is an effec-tive in the treatment of osteoporotic thoracolumbar vertebral fracture nonunion without neurological deficit and obvious kyphotic deformity ,it is worthy of clinical application .
