桃红四物汤预防腰椎骨折术后下肢深静脉血栓的应用

The Application of Taohong Siwu Decoction in Prevention of Deep Venous Thrombosis of Lower Extremity of Lumbar Vertebral Fracture after Operation

目的探究桃红四物汤在腰椎骨折术后患者预防下肢深静脉血栓中的应用效果。方法选取2012年7月—2014年7月于我院进行手术治疗的164例腰椎骨折患者,随机等分成观察组和对照组各82例。给予对照组术后传统抗凝治疗,观察组在对照组的基础上联合应用桃红四物汤。比较两组患者术后下肢深静脉血栓(DVT)发生率以及术后第10天的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)以及D-二聚体(D-D)的含量。结果观察组DVT发生率明显低于对照组,差异有统计学意义(P<0.05);术后第10天两组患者平均APTT差异无统计学意义(P>0.05),而观察组平均PT明显高于对照组,FIB以及D-D含量明显低于对照组,差异有统计学意义(P<0.05)。结论在传统腰椎骨折术后抗凝治疗的基础上加用桃红四物汤可明显改善血液高凝状态,降低术后FIB以及D-D含量,从而降低了DVT的发生率,值得临床推广应用。

Objective To explore the application effect of Taohong Siwu decoction in prevention of deep venous thrombosis of lower extremity in patients with lumbar vertebral fracture after operation. Methods Selecting 164 cases of patients with lumbar vertebral fractures, they were randomly divided into observation group and control group. The control group was treated with traditional anticoagulant therapy after the op-eration, while the observation group was given Taohong Siwu decoction based on the control group. The lower extremity deep vein thrombosis（DVT） occurring rate and postoperative day 10 of prothrombin time（PT）, activation activated partial thromboplastin time（APTT）, fibrin origi-nal（FIB） and D dimer（D-D） content of the two groups were compared. Results The incidence of DVT in the observation group was significantly lower than that of the control group, and the difference was statistically significant（P〈0.05）. The average PT of the observation groupwas significantly higher than that of the control group, FIB and D-D content of the observation group were significantly lower than those of the control group, and the difference was statistically significant（P〈0.05）. Conclusion Taohong Siwu decoction for deep venous thrombosis of lower extremity in patients with lumbar vertebral fracture after the operation can obviously improve the hypercoagulable state of traditional lumbar fractures, reduce postoperative FIB and D-D levels, thereby reducing the incidence of DVT, and is worthy of clinical application.
