摘要
目的观察布地奈德联合复方异丙托溴铵雾化吸入佐治小儿肺炎支原体肺炎的临床疗效。方法将105例肺炎支原体肺炎患儿随机分为对照组52例和观察组53例。对照组予大环内酯类抗生素序贯治疗,观察组在对照组基础上加用布地奈德联合复方异丙托溴铵氧驱动雾化吸入,10d为1个疗程。观察两组疗效、咳嗽缓解时间、体温恢复时间、肺部体征消退时间、第10天胸片改善情况、住院时间及治疗前后炎症指标(C反应蛋白、血沉、乳酸脱氢酶)的变化。结果观察组显效率为73.6%(39/53),高于对照组51.9%(27/52),差异有统计学意义(P<0.05);观察组患儿咳嗽缓解时间、体温恢复时间、肺部体征消退时间及住院时间较对照组均明显缩短,差异有统计学意义(P<0.05);治疗第10天复查胸片,观察组炎症基本吸收例数多于对照组,差异有统计学意义(P<0.05);治疗后观察组C反应蛋白、血沉、乳酸脱氢酶明显低于对照组,差异有统计学意义(P<0.05)。结论布地奈德联合复方异丙托溴铵氧驱动雾化吸入佐治小儿肺炎支原体肺炎,疗效显著,能够明显改善临床症状和体征,缩短治疗时间,值得临床推广应用。
Objective To explore the clinical effect of budesonide combined with compound ipratropium bromide aerosol inhalation in the treatment of mycoplasma pneumonia in children.Methods Totally 105 children with mycoplasma pneumonia were randomly divided into control group(n=52),who received macrolide antibiotic sequential therapy,and observation group(n=53),who were given additional budesonide and compound ipratropium bromide oxygen atomizing inhalation,10 days as a course of treatment.The therapeutic effects were observed and compared between the two groups,including cough relief time,temperature recovery time,regression time of physical signs of the lungs,chest X-ray improvement on the 10 th day,hospital stay and the change of inflammation indexes before and after the treatment(CRP,ESR,LDH).Results The effect rate in the observation group was 73.6%(39/53),and the control group 51.9%(27/52),the difference being statistically significant(P〈0.05).Cough relief time,temperature recovery time,regression time of physical signs of the lungs,and hospital stay were significantly shorter in observation group than in control group(P〈0.05).Chest X-ray on the 10 th day showed that in the observation group inflammation was basically absorbed in more cases than in the control group,the difference being statistically significant(P〈0.05).Inflammation indexes(CRP,ESR,LDH)of the observation group were lower than the control group after treatment,the difference being statistically significant(P〈0.05).Conclusion Budesonide combined with compound ipratropium bromide oxygen atomizing inhalation is effective in the treatment of mycoplasma pneumonia in children.It can obviously improve the clinical symptoms and signs and shorten the treatment time,which is worthy of clinical application.
