急性初发脑梗死患者焦虑抑郁状况与社会支持的相关性研究

Correlation study of anxiety and depression mood and social support among patients with primary brain infarction

目的探讨初发脑梗死患者早期社会支持与心理健康的相关性。方法对2013年4月~2014年3月入住安徽医科大学第一附属医院神经内科的80例初发脑梗死患者应用医院焦虑抑郁量表(HADS)、社会支持评定量表(SSRS)、美国国立卫生院神经功能缺损评分(NIHSS)、自编患者一般资料调查表进行评估,并分析其相关性。结果患者焦虑发生率为33.8%,抑郁发生率为31.3%。社会支持总分为(36.79±7.89)分,支持度一般。60岁及以上初发脑梗死患者社会支持各维度评分与社区慢性病老人比较,差异无统计学意义(P>0.05)。社会支持总分及各维度评分与焦虑(r=-0.114^-0.263,P<0.05)和抑郁均呈负相关(r=-0.201^-0.337,P<0.05)。结论约1/3的初发脑梗死患者存在焦虑抑郁情绪,医护人员若能尽早评估其社会支持情况并努力调动患者支持系统发挥积极作用,将有利于缓解患者的负性情绪。

Objective To explore the relationship between psychological health and social support among patients with primary brain infarction. Methods 80 patients with primary cerebral infarction hospitalized in Department of Neurology of the First Affiliated Hospital of Anhui Medical University from April 2013 to March 2014 were enrolled to give inves- tigation, including hospital anxiety and depression scale （HADS）, social support rating scale （SSRS）, national institutes of health stroke scale （NIHSS） and general information questionnaire. The correlation was analyzed. Results The preva- lence of anxiety and depression among patients was 33.8% and 31.3% respectively. The total score of social support was （36.79±7.89） points and the level was general. There were no significant differences in social support scores of each dimension between patients aged over 60 with primary cerebral infarction and the elders with chronic diseases in communities （P 〉 0.05）. Total score and score of each dimension on social support were all negatively correlated with the score of anxiety （r=-0.114- -0.263, P 〈 0.05） and depression （r=-0.201- -0.337, P 〈 0.05）. Conclusion About 1/3 patients with primary brain infarction are in anxiety and depression mood. If medical staffs can assess the level of patients＇ social support as early as possible and try to mobilize the patients＂ social support system into full play, the negative emotion will be relieved.
