注射用骨瓜提取物对膝骨性关节炎的疗效及膝关节液中IL-1表达的影响

Effect of cervus and cucumis polypeptide injection on the treatment of knee osteoarthritis and its influence on the expression of IL-1 in knee joint fluid

目的探讨注射用骨瓜提取物(益古泰)对膝骨性关节炎(KOA)的疗效及膝关节液中白细胞介素-1(IL-1)表达的影响。方法选取100例KOA患者作为研究对象,将其随机分为观察组和对照组,每组50例。对照组患者给予口服氨基葡萄糖和尼美舒利进行治疗,观察组患者给予静脉滴注注射用骨瓜提取物和口服尼美舒利进行治疗。对2组患者的临床疗效、不良反应及治疗前后的西安大略一麦克马斯特大学骨性关节炎指数量表(WOMAC)评分、膝关节液中的IL-1水平进行观察和比较。结果观察组患者的疗效分布和临床有效率均优于对照组(U=3.603,χ2=6.775,P<0.05),观察组患者治疗后的三个维度WOMAC评分均显著低于对照组(t=4.538、3.645、4.663,P<0.05),膝关节液中IL-1水平显著低于对照组(t=4.377,P<0.05),观察组患者的不良反应发生率低于对照组(χ2=4.891,P<0.05)。结论应用注射用骨瓜提取物治疗KOA,能够有效缓解患者的临床症状和体征,降低关节局部的炎症反应程度。

Objective To explore the therapeutic effect of cervus and cucumis polypeptide injection on the treatment of knee osteoarthritis（ KOA） and its influence on the expression of interleukin-1（ IL-1） in knee joint fluid. Methods A total of 100 patients with KOA were selected as the research objects and randomly divided into observation group and control group,50 cases in each group.The patients in the control group were treated with oral administration of glucosamine and nimesulide,while the patients in the observation group were treated with intravenous cervus and cucumis polypeptide injection and oral nimesulide. The clinical therapeutic effect,the adverse events during the treatment,the Western Ontario Mc Master University Osteoarthritis Index（ WOMAC） score,the IL-1 level in knee joint fluid before and after the treatment were compared between the two groups. Results The distribution of therapeutic effects and the clinical efficiency of the observation group were significantly better than those of the control group（ U = 3. 603,χ2= 6. 775,P〈 0. 05）. The WOMAC scores at three dimensions in the observation group after the treatment were significantly lower than those in the control group（ t = 4. 538,3. 645,4. 663,P〈 0. 05）. The IL-1level in the knee joint fluid was significantly than that in the control group（ t = 4. 377,P〈 0. 05）. The overall incidence rate of the adverse events in the observation group was lower than that of the control group（ χ2= 4. 891,P〈 0. 05）.Conclusion Application of cervus and cucumis polypeptide injection in the treatment of KOA can relieve the clinical symptoms and signs effectively,and reduce the degree of inflammation in joint.
