丁苯酞软胶囊与尼莫地平治疗卒中后血管性痴呆的临床疗效比较

Comparative Study for Clinical Effect on Vascular Dementia after Stroke between Butylphthalide Soft Capsules and Nimodipine

目的比较丁苯酞软胶囊与尼莫地平治疗卒中后血管性痴呆的临床疗效。方法选取新疆医科大学附属中医医院2013年5月—2014年11月收治的卒中后血管性痴呆患者98例,采用随机数字表法分为对照组和试验组,各49例。对照组患者在常规治疗基础上采用尼莫地平治疗,试验组患者在常规治疗基础上采用丁苯酞软胶囊治疗。治疗前后分别采用简易精神状态量表(MMSE)评估患者认知功能,采用日常生活活动能力量表(ADL)评估患者日常生活能力,比较两组患者临床疗效并观察治疗期间不良反应发生情况。结果治疗前两组患者MMSE评分和ADL评分比较,差异无统计学意义(P>0.05);治疗后试验组患者MMSE评分、ADL评分均高于对照组(P<0.05)。试验组患者临床疗效优于对照组(u=3.936,P=0.000)。对照组5例患者出现血压降低。结论丁苯酞软胶囊治疗卒中后血管性痴呆的临床疗效优于尼莫地平,其能明显改善患者认知功能和日常生活能力,且安全性较高。

Objective To compare the clinical effect on vascular dementia after stroke between butylphthalide soft capsules and nimodipine. Methods A total of 98 patients with vascular dementia after stroke were selected in the Affiliated Traditional Chinese Medicine Hospital of Xinjiang Medical University from May 2013 to November 2014,and they were divided into control group and test group according to random number table,each of 49 cases. Based on conventional treatment,patients of control group were given extra nimodipine,while patients of test group were given extra butylphthalide soft capsules. Before and after treatment, MMSE was used to evaluate the cognitive function, ADL was used to evaluate the activities of daily living,clinical effect was compared between the two groups and incidence of adverse reactions was observed during treatment. Results No statistically significant differences of MMSE score or ADL score were found between the two groups before treatment（ P〉0. 05）,while MMSE score and ADL score of test group were statistically significantly higher than those of control group（ P〈0. 05）. The clinical effect of test group was statistically significantly better than that of control group（ u = 3. 936,P = 0. 000）.During treatment,5 cases of control group occurred blood pressure decreased. Conclusion Butylphthalide soft capsules has better clinical effect than nimodipine in treating vascular dementia after stroke,can effectively improve the cognitive function and activities of daily living,is with high safety.
