垂体后叶素联合四物汤加味治疗老年支气管扩张咯血的临床疗效

Clinical Effect of Pituitrin Combined with Modified Siwu Decoction on Elderly Bronchiectasis Patients Complicated with Hemoptysis

目的探讨垂体后叶素联合四物汤加味治疗老年支气管扩张咯血的临床疗效。方法选择恩施土家族苗族自治州民族医院2012年3月—2014年3月收治的老年支气管扩张咯血患者86例,按照随机数字表法分为观察组和对照组,各43例。在常规对症治疗基础上,对照组患者给予垂体后叶素治疗,观察组患者在对照组基础上联合四物汤加味治疗,两组患者均以7 d为1个疗程。比较两组患者临床疗效、临床症状(呼吸困难、气促、咳嗽)缓解时间、咯血停止时间及住院时间,治疗期间观察两组患者不良反应发生情况。结果观察组患者临床疗效优于对照组(P<0.05);观察组患者呼吸困难、气促、咳嗽症状缓解时间及咯血停止时间、住院时间均短于对照组(P<0.05)。观察组患者治疗期间不良反应发生率为9.3%,低于对照组的20.9%(P<0.05)。结论垂体后叶素联合四物汤加味治疗老年支气管扩张咯血的临床疗效确切,能有效改善患者临床症状,缩短咯血停止时间,且安全性较高。

Objective To investigate the clinical effect of pituitrin combined with modified siwu decoction on elderly bronchiectasis patients complicated with hemoptysis. Methods A total of 86 elderly bronchiectasis patients complicated with hemoptysis were selected in the National Hospital of Enshi Tujia and Miao Autonomous Prefecture from March 2012 to March2014,and they were randomly divided into observation group and control group, each of 43 cases. Based on conventional symptomatic treatment,patients of control group were given extra pituitrin,while patients of observation group were given extra pituitrin combined with modified siwu decoction,both groups treated for 7 days as a course. Clinical effect,alleviation time of clinical symptoms（ including dyspnea, anhelation and cough）, hemoptysis stopped time, hospital stays and incidence of adverse reactions during treatment were compared between the two groups. Results The clinical effect of observation group was statistically significantly better than that of control group（ P〈0. 05）. Alleviation time of dyspnea, anhelation and cough,hemoptysis stopped time and hospital stays of observation group was statistically significantly shorter than that of control group,respectively（ P〈0. 05）. The incidence of adverse reactions during treatment of observation group was 9. 3%,was statistically significantly lower than that of control group of 20. 9%（ P〈0. 05）. Conclusion Pituitrin combined with modified siwu decoction has certain clinical effect in treating elderly bronchiectasis patients complicated with hemoptysis,can effectively relieve the clinical symptoms,shorten the hemoptysis stopped time,and is safe.
