Analysis on Mixing Error Reasons of Common Drugs in PIVAS and its Countermeasure

目的:分析静脉用药调配中心(PIVAS)常见药品调配差错原因及改进措施。方法:收集2014年1—12月间PIVAS出现的各类药品调配差错处方,对各类差错处方产生的原因进行分析,并提出改进措施。结果:PIVAS在2014年药品调配差错率为0.046%,主要发生在摆药、贴签、加药、打包与外送等环节。结论:通过建立完善的工作制度、加强人员管理、品管圈工具的应用以及优化智能摆药系统,减少药品调配差错的发生,为患者提供合格的成品输液,保障临床静脉用药的安全有效。

Objective： To analyze the mixing error reasons of common drugs in PIVAS and its countermeasure. Methods： All errors occurring in PIVAS of our hospital in 2014 were collected, and the reasons for the error occurring were explored. The corresponding improvement measures were proposed. Results： The error rate was 0.046% in 2014 in PIVAS of our hospital. The errors were mainly occurred in drug dispensing, labeling, drug admixture, packaging and delivery. Conclusion： By establishing a sound working system, strengthening personnel management, applying quality control circles, and optimizing drug dispensing intelligent system, the errors in each step of intravenous admixture can be reduced, the qualified drug use of patient can be guaranteed and the safety of clinical drug use can be improved.
