卡前列素氨丁三醇联合Foleys导尿管治疗子宫收缩乏力性产后出血的临床效果

Clinical efficacy of carboprost tromethamine combined with Foleys catheter in the treatment of postpartum hemorrhage due to uterine inertia

目的 探讨卡前列素氨丁三醇联合Foleys导尿管治疗子宫收缩乏力性产后出血的的临床效果。方法 选取2013年2月~2015年1月我院收入治疗的子宫收缩乏力性产后出血患者85例,其中对照组40例采用常规治疗（宫腔纱条填塞术）,观察组45例患者采用卡前列素氨丁三醇联合Foleys导尿管填塞宫腔治疗,比较两组患者的术后止血有效率、手术一般情况、再出血率及产褥感染率。结果 观察组患者的止血有效率为95.6%,对照组患者止血有效率为92.5%,两组比较差异无统计学意义（P〉0.05）;观察组患者在止血时间、术中出血量等手术一般情况上均明显优于对照组患者（P〈0.05）;观察组患者产褥感染率为2.2%,取出后再出血率为0,低于对照组患者15.0%的产褥感染率及12.5%取出后再出血率（P〈0.05）。结论 卡前列素氨丁三醇联合Foleys导尿管治疗子宫收缩乏力性产后出血,能快速有效地止血,且较为安全可靠,值得临床应用。

Objective To explore and analyze the clinical efficacy of carboprost tromethamine combined with Foleys catheter in the treatment of postpartum hemorrhage due to uterine inertia. Methods From February 2013 to January 2015,85 patients with postpartum hemorrhage due to uterine inertia admitted into our hospital were selected.In control group（n=40）,conventional treatment,that was intrauterine packing with gauze,was used,while in observation group（n=45）, carboprost tromethamine combined with Foleys catheter for uterine cavity packing was adopted.The effective rate of stanching bleeding after surgery,general condition of surgery,rate of rehaemorrhagia,and rate of postpartum infections were compared between two groups. Results The effective rate of stanching bleeding in observation group was 95.6%, and 92.5% in control group,which didn＇t display a statistical difference （P〉0.05）.The general conditions of surgery like time of hemostasis and intraoperative amount of bleeding was much better than that in control group respectively （P〈 0.05）.The rate of postpartum infections and rate of rehaemorrhagia in observation group were 2.2% and 0,while in con- trol group, they were 15.0% and 12.5% respectively.The great superiorities in observation group were remarkable （P〈 0.05）. Conclusion Combination of carboprost tromethamine and Foleys catheter can quickly and effectively stanch bleeding in the treatment of postpartum hemorrhage due to uterine inertia.It is relatively safe and reliable,and is worthy of clinical application.
