摘要
目的探讨胰岛素样生长因子1受体(IGF-1R)和血管内皮生长因子(VEGF)在肝细胞癌(HCC)中的表达与HCC临床病理特征之间的关系及在HCC预后评估中的意义。方法采用免疫组织化学EnVision两步法检测石蜡包埋HCC病例癌组织及配对癌旁组织中IGF-1R及VEGF的蛋白表达,并计数微血管密度(MVD),结合临床病理特征和随访结果进行分析。结果 (1)IGF-1R、VEGF的蛋白表达水平及MVD计数在癌组织高于癌旁组织(P<0.05);(2)IGF-1R及VEGF的蛋白表达在不同病理分级、TNM分期、有无血管浸润及预后组间比较差异均有统计学意义(P<0.05)。(3)IGF-1R及VEGF的蛋白表达强度与MVD水平、IGF-1R的蛋白表达强度与VEGF的蛋白表达强度间均呈正相关。结论IGF-1R及VEGF促进了HCC的发生和发展且与预后密切相关,可用于其预后评估。
Objective To investigate the expression of insulin‐like growth factor 1 receptor (IGF‐1R) ,vascular endothelial growth factor (VEGF) and their relationships between clinic opathologic features in hepatocellular carci‐noma (HCC) .Method Immunohistochemical envision method was utilized to detect the expression of IGF‐1R and VEGF ,MVD was counted .The follow‐up data of the cases of HCC was analyzed .Result The IGF‐1R and VEGF protein shown a higher expression in HCC than in the tumor‐adjacent tissues (P〈 0 .05) .The difference of the number of IGF‐1R and VEGF protein overexpression in histological grade and TNM stage and prognosis was signifi‐cant (P〈0 .05) ,moreover ,the vascular infiltration group was significantly more than group without vascular inva‐sion (P〈0 .05) .The value of MVD was higher in HCC than that in tumor‐adjacent tissues .By Spearman rank cor‐relation analysis ,the level of MVD was positively correlated with IGF‐1R protein expression and VEGF protein ex‐pression .Moreover ,the expression of IGF‐1R and VEGF protein was also positive correlation .Conclusion IGF‐1R and VEGF protein may promote the occurrence and development of HCC ,and they all promote the neovasculation in HCC .The expression of IGF‐1R and VEGF can be used as a marker to evaluate the prognosis of HCC .
