摘要 目的探讨test-bolus技术在MRI肝动脉多期相扫描中的应用价值。方法搜集行上腹部MRI检查患者50例,采用完全随机化分为2组。经验组25例,采用传统经验性延迟时间扫描法;TB组25例,采用test-bolus技术,测得腹主动脉达峰时间,据此获得扫描延迟时间进行扫描。评价两种方法所得肝动脉三期图像质量并进行比较。结果经验组准确显示肝动脉早中晚三期者仅5例;TB组腹主动脉达峰时间不一,均能准确显示动脉早中晚三期,两组各期成像评分均有统计学差异(P值分别为0.039、0.013、0.047,均<0.05)。虽然经验组肝动脉三期分期准确性低于TB组,但两组均能捕获肝动脉晚期图像(均为100%)。结论采用test-bolus技术可个体化准确捕捉肝动脉多期相;而对于肝动脉晚期的显示,则与传统方法无明显差别。 Objective To explore the value of the test - bolus technique in multiphase scanning MRI of hepatic artery. Methods Fifty patients with abdominal MR examination were randomly divided into two groups. In group A ( n = 25 ) , the scan delay time was set according to experience of operators ;in group B ( n = 25 ) , the scan delay time was determined by test - bolus technique. Two groups of hepatic arterial multiphase images were evaluated and compared. Results Only 5 cases of group A, can display the stages (early, middle and late) of hepatic artery accurately. In group B, the peak time of abdominal aorta can be observed clearly and individually, which promotes the display of hepatic arterial stages. The score of hepatic arterial muhiphase images of two groups was statistically significant ( P 〈 0.05 ). The late stages of hepatic artery were captured ( 100% ) in group A and B, although group A was inferior to group B in staging accuracy. Conclusion It is possible to capture the stages of hepatic arterial phase accurately with test - bolus technique, but there was no significant difference in displaying the late stage of hepatic arterial phase with the traditional method.
