高频超声对髌下脂肪垫损伤的诊断价值 被引量:4

摘要 目的探讨高频超声对髌下脂肪垫损伤诊断的临床应用价值。方法采用病例对照研究对已确诊的髌下脂肪垫损伤患者分别采取MRI和高频超声检查,对比二种方法对髌下脂肪垫损伤检查的准确率,观察髌下脂肪垫损伤患者高频超声图像。结果高频超声检查髌下脂肪垫损伤的准确率(93.7%)明显高于MRI(86.1%),差异具有统计学意义(χ2=4.013,P=0.045)。高频超声图像结果显示,74例髌下脂肪垫患者脂肪垫肿胀、增厚,内部回声减弱,58.1%患者合并膝关节滑膜炎,高频超声图像结果显示膝关节腔积液、滑膜毛糙、增厚;18.9%患者合并膝关节骨性关节炎,超声图像结果显示膝关节软骨面弱回声,回声带变窄、厚度不均、毛糙等。结论高频超声方法在临床上可以应用于髌下脂肪垫损伤的检查,并且较MRI方法准确率高。 Objective: To explore the clinical value of high frequency ultrasound for infrapatellar fat pad injury. Meth- ods : A case-control study was used to evaluate the accuracy of high frequency ultrasound and MRI methods for diagnosis of infrapatellar fat pad injury and observe the features of high frequency ultrasound. Results: The accuracy was significantly higher with high frequency ultrasound method (93.7 % ) than that with MRI method (81.6% ), the difference had statisti- cal significance (x^2 = 4. 013, P = 0. 045 ). High frequency ultrasound figures showed that the pads of 74 patients were swelled and thickened, and the echo was weakened. The figures of 58.1% patients showed articular effusion of knee joint, coarse, synovial thickening when combined with gonarthromeningitis; the figures of 18.9% patients showed weakened ech- o, echo band narrowing, uneven thickness and crudeness when combined with osteoarthritis of the knee. Conclusion: High frequency uhrasound can be used for diagnosing infrapatellar fat pad injury in clinical, and it is more accurate than Mill.
