胃肠道间质瘤的病理学特点分析(附58例报告)

摘要 目的提高对胃肠道间质瘤(gastrointestinal stromal tumors,GIST)的免疫组织化学特征及良恶性诊断方法的认识。方法查阅赤壁市人民医院GIST患者58例的临床病理资料并结合CD 117等抗体结果分析。结果58例手术切除的GIST病例中男性33例,女性25例。GIST原发于胃34例(58.6%),十二指肠4例(6.9%),空肠6例(10.3%),回肠6例(10.3%),结肠2例(3.4%)、直肠4例(6.9%),肠系膜2例(3.4%)。免疫组织化学特征:58例(100.0%)波形蛋白vimentin阳性,55例(94.8%)CD 117阳性,45例(77.6%)CD 34阳性。结论 GIST是最常见表达CD 117抗体阳性的胃肠道的间叶性肿瘤,中老年男性多发。GIST的最有效的诊断方法是光镜下结构结合其免疫标记物阳性表达的CD 117和CD 34。肿瘤腹腔广泛播散,转移远处脏器,肿瘤的浸润性生长方式,肿瘤的大小及原发部位和病理性核分裂象的多少是判定良恶性的重要指针。
