食管小细胞癌47例诊疗及预后分析

Diagnosis/treatment and prognosis of 47 cases of small cell carcinoma of the esophagus

目的探讨食管原发性小细胞癌的临床特征、治疗效果及预后。方法收集2002—01—2012—01大连医科大学附属第一医院收治的47例原发性食管小细胞癌患者的临床资料，分析患者在不同治疗方法下的中位生存期，并对食管小细胞癌患者进行单因素和多因素生存分析，找出食管癌的诱发因素。结果综合中位生存期为25个月，1、2、3和5年生存率分别为53．2％、36．2％、23．4％和10．6％。KaplanMeier分析表明，治疗影响患者的生存，Cox回归模型分析显示，肿瘤分期（P=0．048）和肿瘤的治疗方式（P=0．012）是食管原发性小细胞癌预后的独立影响因子。结论综合治疗手术与放疗和化疗相结合可以提高食管原发性小细胞癌患者的生存率。

OBJECTIVE To investigate the clinical characteristics,therapeutic effect and prognosis of primary small cell carcinoma of the esophagus. METHODS To review the clinical data of 47 cases of primary esophageal small cell carci- noma admitted to our hospital from January 2002 to January 2012 ,packet analysis of survival rate of patients with differ ent treatment methods and median survival time, and analyze the independent image of small cell carcinoma of the esopha- gus factors. RESULTS The overall median survival time was 25 months, the survival rate of 1,2,3,5 were 53. 2%, 36.2% ,23.4%, 10.6%. Kaplan-Meier analysis showed that treatment have influence on patient survival,and Cox regres- sion model analysis showed that tumor stage and treatment are independent risk factors of small cell carcinoma of the esophagus. CONCLUSION The comprehensive treatment of surgical operation combined with radiotherapy and chemotherapy can improve the survival rate of the patients.
