摘要
目的观察原发性肝癌(PHC)患者外周血中辅助性T细胞17(Th17)与细胞因子白介素17(IL-17)的分泌水平,探讨Th17细胞及IL-17与PHC病理发展的关系。方法选取PHC患者80例以同期40例健康体检健康者作为对照组,收集两组的外周血后采用流式细胞术检测Th17细胞亚群的比例,ELISA法检测血清中的IL-17水平,RT-PCR检测转录因子RORγT m RNA水平,并分析RORγT表达水平与PHC临床病理学特征的关系。结果 PHC患者外周血中Th17占CD4+T细胞比例为(4.09±0.33)%,显著高于对照组(1.38±0.27)%,差异有统计学意义(P<0.05);PHC患者外周血中IL-17水平为(57.73±6.21)pg/ml,显著高于对照组(33.98±4.47)pg/ml,差异有统计学意义(P<0.05);PHC患者外周血转录因子RORγT m RNA水平显著高于对照组,差异有统计学意义(P<0.001)。肝癌组织中RORγT m RNA的表达水平与肝癌组织Edmondson病理分级显著相关(r=0.685,P<0.05),且外周血中RORγT m RNA表达水平与HCC分化程度显著正相关(r=0.603,P<0.05)。结论 Th17及IL-17及转录因子RORγT m RNA水平在PHC患者外周血表达增加,且RORγT与PHC的病理分级程度明显相关。
【Objective】 To observe the T helper 17(Th17) cells and interleukin 17(IL-17) level in primary hepatocarcinoma(PHC),and explore the relationship between Th17/IL-17 and pathological development of PHC.【Methods】 A total of 80 cases of PHC were collected as study group and 40 cases of healthy sunjects as control group.Th17 cells in peripheral blood were analyzed by flow cytometry.Serum IL-17 level was detected by ELISA,and the transcription factor ROR γ T m RNA level was detected by RT-PCR.The relationship between PHC ROR γ T and clinicopathological features was also analyzed.【Results】 The percentage of Th17 cells in peripheral blood of the PHC patients [(4.09 ± 0.33)%] was significantly higher than that in the controls [(1.38 ± 0.27)%],the difference was statistically significant(P〈0.05).The IL-17 level in the PHC patients [(57.73 ± 6.21) pg/ml] was also significantly higher than that in the controls [(33.98 ± 4.47) pg/ml,P〈0.05].The level of ROR γ T m RNA in peripheral blood of the PHC patients was significantly higher than that of the controls(P〈0.001).ROR γ T m RNA expression in the PHC tissue was significantly correlated with the Edmondson pathological grade(r = 0.685,P〈0.05),and ROR γ T m RNA expression in peripheral blood was positively correlated with the degree of PHC differentiation(r = 0.603,P〈0.05).【Conclusions】 Th17 cells,IL-17 and ROR γ T m RNA levels are increased in peripheral blood of PHC patients,and ROR γ T m RNA expression is significantly correlated with the pathological grade of PHC,suggesting ROR γ T as latent factor in PHC diagnosis.
