硬性透气性角膜接触镜治疗儿童屈光参差性弱视的临床疗效

Clinical evaluation of rigid gas-permeable contact lens for the treatment of anisometropic amblyopia in children

目的评价长期配戴硬性透气性角膜接触镜(RGPCL)对屈光参差性弱视儿童视功能及角膜生理状况的影响。方法回顾性研究。纳入双眼屈光参差﹥2.0 D的弱视儿童287例,其中89例配戴RGPCL,198例配戴单光框架眼镜。经过2年治疗,比较2组的弱视眼最佳矫正视力、立体视功能,并评估RGPCL组的中央角膜厚度及角膜内皮细胞计数。结果经过2年治疗,RGPCL组弱视眼最佳矫正视力以及远、近立体视明显优于框架眼镜组,差异均具有统计学意义(P<0.05)。中央角膜厚度及角膜内皮细胞计数均未发生显著变化(P>0.05)。结论在儿童远视屈光参差性弱视治疗中,配戴RGPCL比配戴框架眼镜更有利于视力、视功能提高,同时具有很好的安全性。

Objective To evaluate the effect of long-term wear of rigid gas-permeable contact lens （RGPCL） on the visual function and the corneal physiology during the treatment of anisometropic amblyopia in children. Methods This was a retrospective study. A total of 287 amblyopic children who had anisometropia of greater than 2.0 D were included; 89 of them wore RGPCLs and 198 wore single vision spectacle lenses （SVL）. The best-corrected distance visual acuity （CDVA） and stereoacuity were compared between the two groups after 2 years of treatment. And the central corneal thickness and corneal endothelial cell counting were evaluated. Results The CDVA and stereoacuity of the RGPCL group were significantly superior to those of the SVL group after 2 years of treatment and the differences were statistically significant respectively （P 〈 0.05 ）. Central corneal thickness and corneal endothelial cell counting showed no significantly change after long-term wear of RGPCL （ P 〉 0.05 ）. Conclusions RGPCL is superior to SVL in improving visual acuity and visual function in anisometropic amblyopic children with commendable safety.
