缩宫素与COOK双球囊在促进宫颈成熟合并引产的临床对比研究

Clinical comparative study of oxytocin and COOK double balloon in promoting cervical mature combining induced labor

目的：探究静点小剂量缩宫素与采用库克子宫颈扩张球囊（ COOK）双球囊在促进宫颈成熟合并引产的临床效果。方法选取武汉市商业职工医院自2014年5月至2015年5月产科收治的90例孕妇作为研究对象，采用随机数字表法分为两组，使用缩宫素促进宫颈成熟并引产为对照组，使用COOK双球囊促进宫颈成熟并引产为试验组，对两组受检者的宫颈成熟率、引产方式、引产后母婴的相关并发症及产妇产后6个月时性生活质量进行比较分析。结果试验组孕妇引产中宫颈成熟度、阴道分娩率均显著高于对照组（χ^2值分别为6．06、5．54，均P＜0．05），剖宫产率显著低于对照组（χ^2＝5．32，P＜0．05），而两组产后出血量比较无显著性差异（t＝1．48，P＞0．05），宫颈裂伤和阴道血肿发生率比较均无显著性差异（t值分别为2．96、3．04，均P＞0．05）。两组新生儿体重、出生后5min Apgar评分比较均无显著性差异（t值分别为1．56、3．16，均P＞0．05），新生儿窒息率比较无显著性差异（χ^2＝0．00，P＞0．05）。两组产妇产后6个月时性生活满意率、性欲下降率、性交疼痛率比较均无显著性差异（χ2值分别为2．46、2．54、2．32，均P＞0．05）。结论 COOK双球囊可以明显促进宫颈成熟并引产，并且效果明显优于缩宫素，未增加母婴引产后的并相关发症，且对性生活质量无明显影响。

Objective To explore and analyze the clinical effect of static point of small dose of oxytocin and COOK double balloon in promoting cervical mature combining induced labor.Methods From May 2014 to May 2015 90 pregnant women admitted in Business Worker’ s Hospital of Wuhan City were selected as research objects, and they were divided into two groups using random number table method.Control group used oxytocin to promote cervical maturity and induced labor, while experimental group used COOK double balloon. Two groups were analyzed and compared in cervical mature rate, induced labor way, complications associated with induced labor and sexual life quality of puerperas 6 months after labor.Results Compared with the control group, cervical maturity degree and vaginal births rate were significantly higher （χ2 value was 6.06 and 5.54, respectively, both P〈0.05） but cesarean section rate was significantly lower in the experimental group （χ2 =5.32, P〈0.05）.Two groups were not significantly different in postpartum haemorrhage amount （t=1.48, P〉0.05） , and the differences in cervical laceration and incidence of vaginal hematoma were not significant between two groups （ t value was 2.96 and 3.04, respectively, both P〉0.05）.The differences in newborn body weight and 5 min Apgar score were not significant between two groups （t value was 1.56 and 3.16, respectively, both P〉0.05）, and neonatal asphyxia rate was not significantly different （χ2 =0.00,P〉0.05）.There were no significant differences between two groups in postpartum sexual life satisfaction degree after 6 months, decrease rate of sexual desire and painful intercourse rate （χ2 value was 2.46, 2.54 and 2.32, respectively, all P〉0.05）. Conclusion COOK double balloon can obviously promote cervical mature and induced labor, and the effect is better than oxytocin.It does not add maternal and infant complications, and it has no obvious effect on the quality of sexual life.
