糖尿病患者围手术期使用胰岛素泵治疗的临床效果

Perioperative Patients with Diabetes using Insulin Pump Therapy Clinical Effect Observation

目的观察糖尿病患者围手术期使用胰岛素泵治疗的临床效果。方法回顾分析2013年12月—2015年6月该院诊治的100例接受手术治疗的糖尿病患者临床资料,按照围手术期胰岛素不同治疗方案分为两组,将采用多次皮下注射胰岛素治疗的43例患者设为对照组,其余采用胰岛素泵持续皮下注射胰岛素治疗的57例患者设为研究组,对比两组临床治疗情况。结果研究组在两组治疗后血糖指标(FBG、2 h PG及Hb Alc)水平均低于治疗前的基础上,其降低幅度比对照组大,并且术前血糖达标时间、待手术时间、拆线时间及住院时间均显著短于对照组,胰岛素用量也比对照组少,差异有统计学意义(P<0.05);对照组术口感染率17.39%、低血糖发生率23.91%均明显高于研究组5.26%、10.53%,差异有统计学意义(P<0.05)。结论糖尿病患者围手术期使用胰岛素泵治疗的临床效果显著,且使用安全性较高,具有临床实际应用价值。

Objective To observe the perioperative patients with diabetes using insulin pump therapy of clinical effect.Methods A retrospective analysis in December 2013- June 2015 in our hospital diagnosis and treatment of 100 patients who underwent surgical treatment of the clinical data of patients with diabetes, according to different perioperative insulin treatment can be divided into two groups, using multiple subcutaneous injections of insulin treatment of 43 patients were set as control group, the rest by continuous subcutaneous insulin injections of insulin pump therapy of 57 patients were set to the team, compared two groups of clinical treatment. Results The team in the two groups after treatment of blood sugar index（FBG, 2 h PG and Hb Alc） levels are lower than before treatment, on the basis of its lower amplitude is larger than in the control group, and preoperative blood glucose control is at the time, operation time, remove time and hospital stay were significantly shorter than the control group, the insulin dosage is less than the control group, the more significant difference（P〈0.05）; The control mouth infection rate 17.39%, the incidence of hypoglycemia were significantly higher than that of the team were 5.26%, 10.53%, 23.91% more significant difference（P〈0.05）.
