摘要
目的:探讨不同水平细胞角蛋白片段抗原21-1(CYFRA21-1)、糖类抗原125(CA125)、癌胚抗原(CEA)和神经元特异性烯醇化酶(NSE)在肺癌诊断中的价值。方法:采用电化学发光免疫法检测54例肺癌患者、60例健康体检者和60例肺部良性疾病患者的CYFRA21-1、CAl25、CEA和NSE水平,对不同组别和不同肺癌分型患者之间的肿瘤标志物水平进行统计学分析。结果:肺癌组CYFRA21-1、CAl25、CEA、NSE水平均高于良性疾病组和健康组(P<0.05);小细胞癌患者NSE水平显著高于鳞癌、腺癌患者(P<0.05);肺癌组各肿瘤标志物联合检测阳性例数显著高于各肿瘤标志物单个检测阳性例数(P<0.05);联合检测敏感性、准确性均高于肿瘤标志物单项检测(P<0.05)。结论:CYFRA21-1、CAl25、CEA和NSE可作为肺癌患者早期诊断的辅助指标,四者联合检测可提高诊断的准确性和敏感性。
Objective: To investigate the value of cytokeratin fragment antigen 21-1( CYFRA21-1),carbohydrate antigen 125( CAl25),carcinoembryonic antigen( CEA) and neuron specific enolase( NSE) of different levels in the diagnosis of lung cancer. Methods: The CYFRA21-1,CAl25,CEA and NSE levels in 54 patients with lung cancer,60 healthy subjects and 60 patients with benign lung disease were determined by the Electrochemiluminescence immunoassay. The tumor markers between different groups and patients with different types of lung cancer were statistically analyzed. Results: The CYFRA21-1,CAl25,CEA,and NSE levels in the lung cancer group were higher than those in the benign disease group and healthy group( P〈0. 05). The NSE level in patients with small cell carcinoma was significantly higher than that in patients with squamous cell carcinoma and adenocarcinoma( P〈0. 05). The number of positive cases examined by tumor marker combined examination were higher than that determined by tumor marker single examination in the lung cancer group( P〈0. 05). The sensitivity and accuracy of tumor marker combined examination were higher than those of tumor marker single examination( P〈0. 05). Conclusion: CYFRA21-1,CAl25,CEA and NSE can be used as auxiliary parameters in the early diagnosis of lung cancer. The four combined examination can improve the diagnostic accuracy and sensitivity.
