改良型包皮环切术的临床效果观察

Clinical effect of modified circumcision

目的:观察分析包皮环切术整形改良的临床疗效。方法:选取从2012年3月至2014年3月收治的包皮过长和包茎患者共234例。随机将患者分为观察组和对照组。其中观察组患者采用改良包皮环切术,共117例;对照组患者则采用传统包皮环切术共117例。结果:观察组和对照组的手术时间对比没有统计学意义。但观察组的手术出血量以及术后疼痛持续时间则明显比对照组要短,两组数据对比有统计学差异。观察组术后不良反应率为23.1%,对照组的不良反应率为66.7%,两组数据对比有显著性差异。观察组患者的IIEF-5评分在术后评分出现下降,并且对照组患者下降的幅度大于对照组,说明患者术后性功能受到影响,但观察组的影响要少于对照组患者,两组数据对比有显著性差异。而患者在TSS评分方面,对照组对比手术前后改变不大,观察组患者则是在术后患者性生活满意度得到提高,两组数据对比有显著性差异。结论:改良包皮环切术安全、美观,能够有效治疗包皮过长和包茎,可为患者提供满意的临床效果。

Objectives： To observe the clinical efficacy of modified circumcision. Methods： 234 patients with long foreskin and phimosis treated in our hospital from March 2012 to March 2014 were selected. The patients were randomly divided into two groups, control group and observation group. Observation group were treated with modified circumcision, a total 117 eases; the control group were treated with traditional circumcision. Results： Op- erating time of observation group and control group was not significantly different. However, the amount of blood loss and postoperative pain duration of observation group were significantly better than these of control group, with staffs-tically significant difference. Postoperative adverse reaction rates of observation group and control group was 23.1% and 66.7% respectively. The difference was significant. IIEF - 5 scores of observation group were declined in the postoperative scoring. The decrease range in the control group was higher than that of the observation group, which indicated that the postoperative sexual function was affected. But the impact to the observation group was lower than the impact to control group, with significant difference. In terms of TSS scores, before and after surgery, the chan- ges of control group was little. The postoperative sexual satisfaction of observation group was improved, with signifi- cant difference. Conclusion： The modified eireumcision is safe, which can effectively treat long foreskin and phi- mosis and achieve satisfactory clinical results.
