TVT-O手术治疗对妇女尿失禁患者性功能影响的临床研究

Influence of TVT- O surgery on the sexual function of female patients with urinary incontinence

目的:探讨TVT-O手术治疗对妇女尿失禁患者性功能影响。方法:本研究选择的对象为2010年6月至2013年5月我市某三甲医院收治的86例压力性尿失禁患者,根据患者治疗压力性尿失禁的过程中所采取的手术方法的不同将其分为对照组与观察组,其中对照组患者采用无张力阴道悬吊术(TVT),观察组患者采用由内向外经闭孔无张力悬吊术(TVT-O),观察两组患者两种手术治疗方法对患者性功能的影响。结果:对照组与观察组患者的平均年龄进行比较无显著性差异(P>0.05),两组患者的术中出血量、手术时间、术后导尿时间以及住院时间进行比较均有显著性差异(P<0.05);对照组与观察组患者治疗前的性生活质量评分进行比较无显著性差异(P>0.05),治疗1个月、3个月、6个月、12个月后的性生活质量评分行比较有显著性差异(P<0.05);对照组与观察组患者治疗后的性功能状况评分(女性的性欲、性唤起、性高潮、性心理以及性满意度)进行比较有显著性差异(P<0.05)。结论:综上所述,采用TVT-O的手术方式相比于TVT的手术治疗方式,能够有效改善压力性尿失禁患者的不自主遗尿情况,提高患者的生活质量,对患者的性生活质量以及性功能各项指标无显著性的影响,值得对TVT-O的手术治疗方式进行进一步的深入研究。

Objectives： To investigate the impact of TVT - O - surgery on the sexual function of female pa- tients with urinary incontinence. Methods： 86 patients with stress urinary incontinence in our hospital from June 2010 to May 2013 were selected and divided, according to the surgical treatment methods, into control group and observation group. The control group was treated with tension - free vaginal tape （ TVT）, and the observation group was treated by tension - free vaginal tape obturator technique （ TVT - O）. The sexual function of the two groups were recorded and compared. Results： The mean age of patients in the control group and the observation group showed no significant difference （P 〉 0. 05 ） ; the blood loss, operative time, postoperative catheterization time and hospital stay time were significantly different between the two groups （ P 〈 0. 05 ） ; scores on sexual life quality be- fore treatment were not significantly different between the two groups （ P 〉 0.05 ）, while scores assessed a month, 3 months, 6 months and 12 months after treatment were significant different （P 〈 0. 05） ; difference in the functional status score （women＇s sexual desire, arousal, orgasm, sexual psychology and sexual satisfaction） were statistically significant between the control group and observation group after treatment （P 〈 0.05）. Conclusion： In summary, compared with TVT, the use of TVT - O can effectively improve the involuntary enuresis in patients with stress uri- nary incontinence and improve their life quality. Besides, it has no significant impact on patient＇ s life quality and sexual function indicators, hereby worthy further in - depth study.
