经腹与经阴道超声检查在异位妊娠中的诊断效果探讨

Effect of different ways of ultrasound diagnosis on ectopic pregnancy

目的:分析并探讨经不同超声检查方式诊断异位妊娠的临床价值。方法:随机选取2010年11月至2014年11月期间在我院接受检查的异位妊娠患者120例。120例患者均采用经腹部B超检查以及经阴道B超检查。对比两种超声检查方法的诊断符合率以及超声图像。结果:经腹部B超符合诊断88例,不符合诊断32例,符合诊断率为73.33%;经阴道B超符合诊断116例,不符合诊断4例,符合诊断率为96.67%,经阴道B超检查方式符合诊断率高于经腹部B超检查方式,经统计学检验,差异具有统计学意义(P<0.05)。经腹部B超检查附件包块97例(80.83%),胚芽7例(5.83%),心管搏动7例(5.83%),盆腔积液76例(63.33%),假孕囊5例(4.17%),经阴道B超检查附件包块118例(98.33%),胚芽25例(20.83%),心管搏动19例(15.83%),盆腔积液107例(89.17%),假孕囊18例(15.00%),经阴道B超超声图像检出率高于经腹部B超,经统计学检验,差异有统计学(P<0.05)。结论:经阴道B超诊断异位妊娠准确率较高,且具有安全无痛等特点,临床上值得推广。

Objectives： To analyze and explore the clinical value of different ways of ultrasound diagnosis on ectopie pregnancy. Methods： 120 patients with ectopic pregnancy treated in our hospital from November 2010 to November 2014 were randomly selected. 120 patients were all tested by transabdominal and transvaginal B - ultra- sound. Diagnose accordanee rate and ultrasound images of the two ways were compared. Results： The diagnosis ac- cordance number of transabdominal B - ultrasound was 88, with a diagnosis rate of 73.33%. Diagnosis accordance number of transvaginal B - ultrasound was 116, with a diagnosis rate of 96.67%. The difference was statistically significant （ P 〈 0.05 ）. Through the transabdominal B - ultrasound, there were 97 cases of adnexal mass （80.83%）, 7 eases of germ （5. 83%）, 7 cases of beating heart tube （5.83%）, 76 cases of pelvic fluid （63. 33% ） and 5 cases of false pregnancy sac （4.17%）. Through transvaglnal B - ultrasound, there were 118 ca- ses of adnexal mass （98.33%）, 25 cases of germ （20. 83% ） , 19 cases of beating heart tube （ 15.83% ） , 107 ca- ses of pelvic fluid （89.17%） and 18 eases of false pregnancy sac （ 15.00% ）. Transvaginal B - ultrasound image detection rate was higher than transabdominal B - ultrasound, with statistically significant difference （ P 〈 0.05 ）. Conclusion： Transvaginal B - ultrasound diagnosis of ectopic pregnancy has features of high accuracy, safety and painless, which is worth popularizing clinically.
