基于128例3年应用资料分析比林斯自然避孕法在社区妇女保健中的应用

Application of Billings natural contraception method in community maternal health care: 3- year data of 128 cases

目的:通过对于社区妇女保健3年的应用资料的整理分析,探讨比林斯自然避孕法的应用效果。方法:选择我区自2010年开始使用比林斯自然避孕法进行避孕,且按时完成3年应用的已婚育龄女性128例,对其应用3年,观察避孕效果,以及采用HAMA进行焦虑情绪的评定,采用SF-36进行生活质量评定。结果:所有研究对象中,共有42例(32.81%)停用比林斯自然避孕法,其中出现3例(2.34%)意外妊娠者;有27例(21.09%)应用至绝经期后停用;有12例(9.38%)认为本法过于复杂,属于方法学停用,续用者89例(69.53%)。共计使用7824个月,其中因意外妊娠及嫌该法复杂者停用10例,使用时间未超过36个月,其余均超过36个月。与应用半年比较,应用1年研究对象的HAMA评分总分及各因子分均降低明显,生活质量指标PF、RP、RE得分均升高明显,差异具有统计学意义(P<0.05)。结论:比林斯自然避孕法避孕效果较佳,可较好的降低妇女的焦虑情绪,提高生活质量,适宜于基层推广应用。

Objectives： To discuss the application of billings natural contraception methods based on the 3 - year data of community maternal health care. Methods ： 128 married women of childbearing age who adopted bill- ings natural contraception method for 3 years from 2010 were followed up in our community. The contraception effec- tiveness was observed, and their anxiety emotion and life qualities evaluation was evaluated by HAMA and SF -36 respectively. Results： Among all the research objects, 42 cases （32.81%） stopped billings natural contraception method, and 3 （2.34%） of them had unintended pregnancy; 27 cases （21.09%） adopted billings natural contra- ception method until menopause; 12 cases （9. 38% ） considered it too complex and stopped, while 89 cases （69.53%） continued to use. Totally, the application was 7824 months. 10 cases discontinued whose usage time was less than 36 months, but others all used it for over 36 months. Compared with those used for half a year, the HAMA total scores and each factor scores of those had used for a year decreased obviously, but the life qualities＇ indexes PF, RP, RE scores all obviously increased, with statistically significant difference （ P 〈 0. 05 ）. Conclu- sion： Billings natural contraception method is of good contraception effectiveness, which can relatively decrease fe- male＇ s anxiety and improve their life qualities, hereby is suitable to promotion and application in primary hospitals.
