儿童消化道异物277例分析

Analysis of 277 gastrointestinal foreign body cases in children

目的总结小儿消化道异物特点,探讨小儿消化道异物适宜的治疗方法。方法回顾性分析2006年8月至2014年9月我院因消化道异物住院治疗患儿的临床资料。结果本组277例。取出异物262例(94.6%),自行排出或放弃治疗15例(5.4%)。内镜取出异物245例(93.5%),手术取出异物17例(6.5%)。异物位于食管206例(78.6%),胃内32例(12.2%),肠道21例(7.6%),消化道外3例。取出异物37种,可见金属利器50例(19.1%),食物碎块30例,硬币27例,电池22例。结论儿童消化道异物诊断及治疗难度较大,需综合考虑病史、临床症状、异物形状、种类、体积、嵌顿时间、停滞部位选择适宜方法。

Objective To analyze the characteristics of children with gastrointestinal foreign bodies and explore the appropriate treatment for gastrointestinal foreign matter in children. Methods The medical record of patients with gastrointestinal foreign bodies in our hospital were analyzed retrospectively. Results From August 2006 to September 2014,277 patients in total were enrolled into the study. 245（93.5%）cases were admitted for endoscopy, in which 206（78.6%）cases had esophagus foreign body, stomach foreign body in 32（12.2%）, intestinal foreign body in 21（7.6%）. The most common foreign body ingested were sharp metal 50（19.1%）. Conclusion Diagnosis and treatment of gastrointestinal foreign body is difficult in children. History, clinical symptoms, shape, type, size, incarceration time of foreign body should be considered when making treatment plan.
