博来霉素碘化油乳剂超选择子宫动脉栓塞术与子宫肌瘤剔除术治疗子宫肌瘤的研究

Contrast study on the effect of treating hysteromyoma with bleomycin lipiodol emulsion ultraselective uterine arterial embolization and uterine leiomyoma resection

目的评价博来霉素碘化油乳剂超选择子宫动脉栓塞术治疗子宫肌瘤的临床疗效、风险及并发症。方法收集87例子宫肌瘤患者临床资料,对比子宫动脉栓塞术与子宫肌瘤剔除术两种方法改善临床特征、子宫及子宫肌瘤大小、手术时间、术中出血量、术后恢复日常生活时间及并发症发生率等多个维度的优劣。结果两种治疗方法均能有效地缩小子宫及子宫肌瘤大小(P<0.05),并能显著改善子宫肌瘤的临床症状(P<0.05),但子宫动脉栓塞术组具有手术时间短、术中出血少、术后恢复时间短及并发症发生率小等诸多优点(P<0.05)。结论博来霉素碘化油乳剂超选择子宫动脉栓塞术具有疗效好、安全性高、副作用小、创伤小的短期疗效优势,值得临床进一步推广运用研究。

Objective To evaluate the clinical efficacy,complications and risks of superselective uterine artery embolization with bleomycin lipiodol emulsion for the treatment of uterine fibroids. Methods We collected clinical datas of 87 cases of uterine fibroids patients,comparing the pros and cons of the methods of uterine arterial embolization and uterine fibroids removal in the improvement of clinical features,uterine and uterine fibroids size,operation time,intraoperative bleeding,postoperative recovery of daily life and complications and other dimensions. Results These two kinds of treatment can effectively reduce the size of the uterus and uterine fibroids（ P〈0. 05）,and can significantly improve the clinical symptoms of uterine fibroids（ P〈0. 05）,but the method of uterine artery embolization had the advantage of shorter operative time,less intraoperative bleeding,less postoperative recovery time and complications（ P〈0. 05）. Conclusions Superselective uterine artery embolization with bleomycin lipiodol emulsion has the advantages of better effect,higher safety,less side effects and smaller trauma,it is worth of further promoting and using in clinical practices.
