腹腔镜结直肠癌根治术与传统根治术的临床效果对比

Comparison of the clinical efficacy of laparoscopic and open radical resection of colorectal cancer

目的:对比腹腔镜结直肠癌根治术与传统开腹手术的临床疗效,评价腹腔镜结直肠癌根治术的可行性、安全性及有效性。方法:选择2011年4月至2015年4月接受治疗的49例结直肠癌患者作为研究对象,其中27例行腹腔镜手术(腹腔镜组),22例行传统开腹手术(开腹组)。对比两组患者手术时间、术中出血量、术后胃肠功能恢复时间、住院时间、复发情况。结果:腹腔镜组术中出血量、手术时间、术后自主排气时间、进食时间及住院时间优于开腹组,差异有统计学意义(P<0.05);腹腔镜组术后生存质量评分显著优于开腹组(P<0.05)。结论:熟悉腹腔镜手术的操作技巧,严格掌握手术适应证,腹腔镜辅助结直肠癌手术是安全、可靠的,疗效确切,患者创伤小,术中出血量少,手术时间短,术后康复快,可作为首选术式。

Objective： To compare the efficacy of radical resection of colorectal carcinoma by laparoscopy versus open approach,and investigate the feasibility,safety and efficacy. Methods： Forty-nine patients with colorectal cancer from Apr. 2011 to Apr.2015 were selected as the research object. 27 patients underwent laparoscopic surgery,and 22 patients received traditional laparotomy.The operation time,bleeding volume,postoperative recovery time of gastrointestinal function,hospital stay and recurrence of the two groups were compared. Results： The intraoperative blood loss,operating time,postoperative independent exhaust time,eating time and hospital stay of laparoscopic group were significantly better than those of laparotomy group（ P〈0. 05）. Postoperative score of life quality in laparoscopic group was significantly better than that in open group（ P〈0. 05）. Conclusions： With skilled laparoscopic technique and strictly controlled indications,laparoscopic assisted colorectal cancer surgery is safe and reliable,has advantages of definitely curative effect,little trauma and blood loss,short operation time,quick recovery,is the first choice of operation.
