喀喇昆仑山不同海拔高度习服人群吸氧效率比较分析

摘要 目的:探讨不同流量氧疗对移居不同海拔高原3个月以上青年血氧饱和度(SaO2)、心率(HR)的影响,观察海拔高度对吸氧效率的影响,为指导长期移居高原人群科学合理吸氧寻求依据。方法:利用掌式血氧饱和度仪,对进驻海拔1 400(平原组,n=17)、3 700(n=25)、4 300(n=26)和5 400m(n=34)的102例男性青年,以不同流量吸氧后的SaO2、HR的变化进行监测对比。结果:随海拔高度的升高,高原习服人群SaO2下降、HR加快(P〈0.05,P〈0.01)。海拔3 700、4 300、5 400m组分别采用2、3、4L/min流量吸氧后SaO2可恢复至平原水平,海拔3 700和4 300m组采用1L/min、5 400m组采用2L/min流量吸氧后HR可恢复至平原正常,与平原组吸氧前比较差异不显著(P〉0.05)。结论:随海拔高度的增加,吸氧效率相应下降。考虑到用氧效果和部队实际,建议保健氧疗在海拔5000m以下采用低流量(2-3L/min),在海拔5000m以上采用中等流量(4L/min)吸氧,必要时监测SaO2水平。 Objective:To investigate the effect of different flow oxygen inhalation on oxygen saturation(SaO2)and heart rate(HR)of youth in different altitude for more than three months,and observe the efficiency of oxygen inhalation in different altitudes,to instructe scientific reasonable oxygen inhaling for people migrated in the plateau.Methods:A total of 102 males were devided into four groups,control group(n=17,1 400 m),3 700 m group(n=25),4 300 m group(n=26),and 5 400 m group(n=34),respectively.SaO2 and HR were measured through the different flow oxygen inhalation in four groups.Results:With the altitude increased,the SaO2 was reduced and HR increased during the acclimatizion to high altitude(P〈0.05,P〈0.01).In the altitude of 3 700 m,4 300 mand 5 400 m,took in oxygen with the different speed of 2L/min,3L/min and 4L/min respectively can recover SaO2 of 1 400 maltitude.And a normal heart rate can be made by taking in the oxygen with the speed of 1L/min in 3 700 mand 4 300 m altitude,also 2L/min in 5 400maltitude(P〈0.05).Conclusion:Oxygen inhalation efficiency was reduced as the altitude raise up,we suggeste that take in oxygen with the flow of 2-3L/min below the altitude of5000 mand 4L/min above instead,it is necessary to monitor the SaO2.
