摘要
目的探讨分析老年2型糖尿病患者的甲状腺激素水平的变化和临床意义。方法整群选取该院2013年11月—2014年11月间前来就诊的54例老年2型糖尿病患者作为研究组,同期体检的54例非糖尿病健康老年人作为对照组,检测两组的三碘甲状腺氨酸(TT3)、甲状腺素(TT4)、游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)、反三碘甲腺原氨酸甲状腺激素(r T3)、促甲状腺激素(TSH)及抗甲状腺过氧化物酶抗体(TPOAb)、抗甲状腺球蛋白抗体(TGAb)水平。结果研究组患者游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)及促甲状腺激素(TSH)均低于对照组水平,反三碘甲腺原氨酸甲状腺激素(r T3)、抗甲状腺过氧化物酶抗体(TPOAb)、抗甲状腺球蛋白抗体(TGAb)均高于对照组水平,差异具有统计学意义(P<0.05)。结论老年2型糖尿病患者甲状腺激素偏离正常水平,二者相互作用可导致病情恶性循环进展,因此对老年2型糖尿病患者进行甲状腺激素水平监测、干预具有重要的临床意义。
Objective To analyze the change of thyroid hormone level in elderly patients with type 2 diabetes and its clinical significance. Methods 54 elderly patients with type 2 diabetes admitted in our hospital during November 2013 and November 2014 were assigned to the study group, and 54 healthy elderly without diabetes in the same period were assigned to the control group.Total triiodothyronine(TT3), total thyroxine(TT4), free triiodothyronine(FT3), free thyroxine(FT4), reverse triiodothyronine(r T3),thyroid stimulating hormone(TSH), antithyroperoxidase antibody(TPOAb) and anti-Thyroglobulin antibody(TGAb) level of the patients were determined in the two groups. Results FT3, FT4, TSH level were lower and r T3, TPOAb, TGAb level higher in the study group than in the control group, and the differences were statistically significant,P〈0.05. Conclusion The combination of type 2 diabetes and abnormal thyroid hormone level can lead to disease progression, therefore it is of clinical significance to deter-mine the thyroid hormone level for elderly patients.
