显微外科手术治疗中央回区脑膜瘤

Microsurgical in Treatment of Meningiomas in Central Gyrus Region

目的：探讨中央回区脑膜瘤的手术方法和治疗效果。方法回顾分析应用显微外科手术切除该院于2008-2013年收治的52例中央回区脑膜瘤的临床资料。结果SimpsonⅠ级切除30例，Ⅱ级切除19例，Ⅲ级切除3例。无手术死亡病例。术后功能障碍好转41例，无效2例，新增功能障碍9例，随访3~36个月，未见肿瘤复发。结论中央回区脑膜瘤应在确保功能的前提下，争取达到SimpsonⅠ级切除。仔细的术前评估，熟练的显微外科技术，注意中央区皮层脑组织的保护，维持静脉回流的通畅，正确的处理矢状窦是降低中央回区脑膜瘤手术并发症、提高治疗效果的关键所在。

Objective To discuss the surgical methods and therapeutic effect of meningiomas in central gyrus region. Methods A retrospective analysis was performed on 52 cases of meningiomas in central gyrus region from 2008-2013. Results SimpsonⅠ, II, and Ⅲ grade removal was achieved in 30,19 and 3 respectively.None of the patients died. Preoperative dysfunction was improved in 41 cases, no functional improvement in 2 cases, additional dysfunction in 9 cases. All the patients were followed up for 3 to 36 months, and there was no recurred case. Conclusion The surgical treatment of meningiomas in central gyrus region should be in good function under the premise of protection and achieve SimpsonⅠgrade removal. Careful preoperative evaluation and skillful microsurgical technique, the protection of the central cortex brain, maintain the venous return unobstructed, the correct processing sagittal sinus are the keys to reduce complications and improve therapeutic effect in meningiomas in central gyrus region.
