艾滋病经高效抗逆转录病毒治疗后发生免疫重建综合征并发肺结核的临床特点研究

Study on Clinical Characteristics of AIDS HAART Treated Immune Recon-struction Syndromecomplicated with Pulmonary Tuberculosis

艾滋病高效抗逆转录病毒治疗虽然有较为满意的临床效果,但治疗后也常会出现免疫重建综合征(IRS)合并肺结核,危及患者生命,影响治疗预后效果。多数高效抗逆转录病毒治疗后IRS合并肺结核者治疗前便伴随肺结核及呼吸道症状,治疗后IRS伴随肺结核者多有纵膈淋巴结重大、肺部结节性病变现象,偶有空洞性病灶、痰菌阳性率不高。该研究为寻求艾滋病实际治疗方案,对艾滋病经高效抗逆转录病毒治疗后发生免疫重建综合征并发肺结核的临床特点进行了综述,高效抗逆转录病毒治疗后的艾滋病IRS伴肺结核患者没有典型症状表现及影像学特点,有较大诊疗难度,应当进一步深入研究力度并扩大研究范围,尽快破解诊疗难题。

AIDS highly active anti retroviral therapy often appear after immune reconstitution syndrome （IRS） complicated with pulmonary tuberculosis. The majority of highly active antiretroviral therapy after IRS combined with pulmonary tuberculosis treat-ment with pulmonary tuberculosis before and after the treatment of IRS with respiratory symptoms, pulmonary tuberculosis were more significant, nodular pulmonary lesions phenomenon of mediastinal lymph node, occasionally cavitary lesions, sputum positive rate is not high. After treatment of IRS withpulmonary tuberculosis in AIDS patient does not have typical symptoms and imaging features,diagnosis and treatment should have greater difficulty, further research efforts and expand the scope of the study.
