血清铁蛋白/总铁结合力比值与冠脉狭窄程度的相关性

摘要 目的:体内铁负荷水平与冠心病的发病有一定的相关性,铁负荷与冠心病的其他危险因素在动脉粥样硬化过程中可能具有一定协同作用。本研究通过测定研究对象血清铁蛋白(SF)及总铁结合力(TIBC)浓度来探讨SF/TIBC比值与冠心病发病之间的关系,及该比值与冠心病严重程度相关性。方法:对185例新入院疑似冠心病患者行冠状动脉造影检查,根据Gensini评分系统对冠脉造影结果评分,将观察对象按照Gensini评分结果分为正常冠脉对照组和冠状动脉轻、中、重度病变组,分别检测各组观察对象血清TIBC、SF、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白-胆固醇(HDL-C)、低密度脂蛋白-胆固醇(LDLC)、血红蛋白(HB)浓度,应用Kruskal-Wallis H检验、Student-Newman-Keuls检验、Tamhane'T2检验和Spearman等级相关分析对上述资料进行分析。结果:各组间LDL-C、HDL-C、SF、TIBC、SF/TIBC比值分布差异有统计学意义(P<0.05),SF/TIBC比值与Gensini积分呈正相关(r=0.589,P<0.01)。结论:SF/TIBC比值与冠心病的发病有一定相关性,与冠状动脉狭窄程度呈正相关。 Objective To study the relationship between serum ferritin( SF)-to-total iron binding capacity( TIBC) ratio and the severity of coronary artery stenosis. Method All of 185 patients suspected coronary heart disease( CHD) underwent coronary angiography and were allocated into control group,mild,moderate and severe group of coronary artery stenosis according to the Gensini score. Levels of SF,TIBC,total cholesterol( TC),triglyceride( TG),high density lipoprotein cholesterol( HDL-C),low density lipoprotein cholesterol( LDL-C),hemoglobin( HB) were examined. The above indexes of the four groups were analyzed by SPSS statistical software and the relationship among SF / TIBC ratio and Gensini score was further investigated by Spearman's rank correlation analysis. Results The distributions of LDL-C、HDL-C、SF、TIBC、SF / TIBC ratio among the four groups were significantly different( P〈0. 05). Spearman's rank correlation analysisshowed that SF / TIBC ratio was positively correlate with the Gensini score( r = 0. 589,P〈0. 01). Conclusion SF / TIBC ratio has a correlation with CHD and is positively correlate with the the severity of coronary artery stenosis.
