128层与640层CTA诊断冠状动脉支架内腔再狭窄的临床对比研究

Clinical comparative study of 128 layers and 640 layers CTA diagnosis of coronary artery stent restenosis lumen

目的比较128层与640层CTA诊断冠状动脉支架内腔再狭窄不同价值，以期寻找最佳的方法，提高诊断准确率。方法选取20i0年5月-2013年5月180例冠状动脉支架植入患者为研究对象，分成2组，每组均为90例，分别予128层和640层CTA检查，观察图像质量和支架内腔显示率，然后以选择性冠状动脉造影术为金标准，比较两者的敏感性、特异性和准确度。结果两者在0分、0．5分、1分、1.5分、2分、2．5分、3分发生率上比较差异有统计学意义（P〈0．05）。128层CTA和640层CTA和选择性冠状动脉造影术比较差异有统计学意义（P〈0．05）；两者在灵敏度、阴性预测值、阳性预测值比较差异无统计学意义（P〉0.05），在准确度、特异性比较差异有统计学意义（P〈0.05）；在狭窄程度上比较差异均无统计学意义（P〉0．05）。结论128层CTA较640层CTA诊断冠状动脉支架内腔再狭窄更加显著，在诊断中意义更大。

Objective To compare the 128 level and 640 level CTA diagnosis of coronary artery stent restenosis lumen different values in order to find the best methods to improve diagnostic accuracy. Methods May 2010 May 2013 180 cases of coronary stent implantation in patients in this study were divided into 2 groups were 90 cases,respectively,and 640 to 128 layers layer CTA exami- nation,observation image quality and brackets lumen show rate, then selective coronary angiography as the gold standard, the sensitivity, specificity and accuracy of comparison between the two. Results Both at 0,0.5,1, l. 5 points, 2 points, 2.5 points, 3 points on the difference between the incidence was statistically significant （P〈0.05）. 128 layers and 640 layers CTA CTA and selective coronary angiography patients was statistically significant difference （P〈0.05） ;both sensitivity and negative predictive value, positive predictive value,the difference was not statistically significant （P〉0.05） in accuracy, specificity was statistically significant difference （P〈0.05） ;the difference in the degree of stenosis was not statistically significant （P〉0.05）. Conclusion 128 compared with 640 layers layer CTA CTA diagnosis of coronary stent lumen restenosis is more significant,more meaningful in the diagnosis.
