茵陈蒿汤联合腺苷蛋氨酸治疗妊娠期肝内胆汁淤积症临床观察

Clinical Observation on Yinchenhao Decoction Combined With Ademetionine in the Treatment of Intrahepatic Cholestasis of Pregnancy

目的观察茵陈蒿汤联合腺苷蛋氨酸治疗妊娠期肝内胆汁淤积症的临床效果。方法随机双盲法将80例妊娠期肝内胆汁淤积症患者分为两组,各40例,对照组采取腺苷蛋氨酸治疗,观察组在对照组基础上加以茵陈蒿汤口服,比较两组临床疗效、不良反应、妊娠结局、治疗前后瘙痒评分及肝功能指标。结果观察组治疗总有效率、早产及新生儿窒息发生率分别为95.0%、10.0%、5.0%,较对照组的77.5%、35.0%、20.0%差异有统计学意义(P<0.05)。观察组治疗后瘙痒评分(0.8±0.4)分,低于对照组的(1.7±0.7)分,差异有统计学意义(P<0.05)。两组治疗后AST、TBA及ALT比较差异显著(P<0.05),均无明显不良反应发生。结论茵陈蒿汤联合腺苷蛋氨酸治疗妊娠期肝内胆汁淤积症安全有效,能降低早产及新生儿窒息发生率。

Objective To observe the clinical effect of Yinchenhao Decoction combined with ademetionine in treatment of intrahepatic cholestasis of pregnancy.Methods 80 patients with intrahepatic cholestasis of pregnancy were divided into two groups by random and double blind method, with 40 cases in each group. The control group was treated with ademetionine, while the observation group, based on the control group, was treated with oral Yinchenhao Decoction. The clinical efifcacy, adverse reactions, pregnancy outcome, pruritus scores and liver function indexes before and after the treatment were compared between the two groups. ResultsThe total effective rate, incidence rates of premature delivery and neonatal asphyxia in the observation group were 95.0%, 10.0% and 5.0%, respectively. Compared with those in the control group （77.5%, 35.0% and 20.0%）, the differences were statistically signiifcant （P〈0.05）. After the treatment, pruritus score of the observation group （0.8±0.4） was signiifcantly lower than that of the control group （1.7±0.7） （P〈0.05）. The differences of AST, TBA and ALT between the two groups after treatment were significant （P〈0.05）. There were no obvious adverse reactions occurring in the two groups.Conclusion Yinchenhao Decoction combined with ademetionine in treatment of intrahepatic cholestasis of pregnancy is safe and effective, which can significantly reduce the incidence of premature delivery and neonatal asphyxia.
